手术和α-受体阻滞剂对BPH患者的疗效对比研究

摘　　要：目的　比较手术和α-受体阻滞剂在不同尿动力学特点 BPH患者中的疗效差别。方法　随访 181例治疗后患者 ,根据其尿动力学特点分为 6组 ,通过分析治疗前后“国际前列腺症状评分”(I- PSS)和“生活质量评分”(QOL )的变化 ,比较手术和α-受体阻滞剂的疗效差别。结果　 1BOO(+)无逼尿肌收缩力减弱 (A)组、BOO(+)有逼尿肌收缩力减弱 (B)组 ,以及 BOO(± )无逼尿肌收缩力减弱患者 (C)组 ,患者手术治疗疗效明显优于药物治疗疗效 (P分别 <0 .0 0 1,0 .0 5 ,0 .0 1) ;BOO(- )无逼尿肌收缩力减弱 (E)组患者的药物疗效明显优于手术疗效 (P<0 .0 5 ) ;BOO(± )有逼尿肌收缩力减弱 (D)组和 BOO(- )有逼尿肌收缩力减弱 (F)组患者对两种治疗方式的反映无明显差别 (P>0 .0 5 )。2 A、B、C组手术对梗阻和刺激症状的改善均优于α受体阻滞剂 ,且手术对梗阻的改善优于其对刺激症状的改善 (P<0 .0 1)。 D组手术治疗对梗阻症状的改善优于α受体阻滞剂 (P<0 .0 5 ) ,但对刺激症状的改善并不优于α受体阻滞剂 (P>0 .0 5 )。E和 F两组α受体阻滞剂对刺激症状的改善明显好于手术治疗 ,且优于它对梗阻症状的改善 (P<0 .0 5 )。结论　 BOO(+)患者应早期手术治疗 ;BOO(± )伴有逼尿肌收缩力减弱的患者仍应手术治疗。Objective To evaluate the effects of operation and α blockade for the therapy of BPH.Methods Compared the IPSS and QOL score of 181 cases of BPH before and after therapy,whom were subdivided into 6 groups according their urodynamic results.Results ① The effects of operation was better than that of α blockade in BOO(+)cases (group A,B)and BOO(±) without decreased contractility patients(group C)( P <0.001,0.05,0.01 respectively);There were no differences between BOO(±) with decreased contractility group (group D)and BOO(-) with decreased contractility group (group F).②Compared with α blockade,the effects of operation on obstructive symptoms were better than on irritating symptoms in group A,B and C( P <0.01);In group E and F,however,the effects of operation on obstructive symptoms were worse than on irritating symptoms ( P <0.05);in group D,operation has better effects on obstructive symptoms,α blockade has better effects on irritating symptoms( P <0.05).Conclusion BOO(+)patients and BOO(±)with decreased contractility patients should receive operation in early stage.
