摘　　要：目的 探讨老年手术患者在麻醉和手术围期白细胞表面黏附分子表达情况。方法 择期髋部手术老年患者２０例，年龄７５～８８岁，采用流式细胞仪方法，在围术期多个时间点测定外周血白细胞黏附分子的表达。结果 与麻醉前相比，手术结束时单核细胞系 ＣＤ１１ｂ，ＣＤ１８，ＣＤ１１ｂ／ＣＤ１８及粒细胞系 ＣＤ１１ｂ的阳性表达率显著增加（Ｐ＜０．０５）；术后第１天单核细胞及粒细胞系的 ＣＤ１１ｂ，ＣＤ１８，ＣＤ１１ｂ／ＣＤ１８的阳性表达率进一步升高，而淋巴细胞系的 ＣＤ１１ｂ，ＣＤ１８，ＣＤ１１ｂ／ＣＤ１８的阳性表达率则较麻醉前显著降低（Ｐ＜０.０１）；术后第３天三者均来完全恢复到术前水平。结论 老年手术患者术后多个细胞系的β２整合素的表达发生显著变化，提示全身免疫功能发生异常改变。Objective To explore the changes of leukocyte adhesion molecule expression in elderly patient perioperatively. Methods 20 patients undertaken hip procedure , aged from 75 to 88 years old, were enrolled in the study. The expression of β2 - in-tegrin (CD11b , CD18 ,CD11b/CD18) in peripheral leukocytes were determined by flow cytometry method Blood sample were taken before anesthesia , at the end of surgery, on the first and third day after operation. Results When compared with the pre - anesthesia , the expression rate of CD11b , CD18 ,CD11b/CD18 on moncytes and CD11b on granulocytes increased significantly at the end of surgery (P < 0.05), and these expression further increased on the first day after operation Whilt the expression of CD11b, CD18, CD11b/CD18 on lymphocytes decreased significantly at the end of operation and on the first day after operation( P < 0 .01). The changes in expression did not completely returned to the preoperative level on the third day after operation. Conclusions Expression of leukocyte β2 - integrin in elderly patients changes significantly at the end surgery and in early days postoperatively, which suggests profound changes in immunological function.
