克雷伯杆菌下呼吸道感染的药物敏感性研究

摘　　要：目的　总结下呼吸道感染的克雷伯杆菌的菌株类型及其对常用抗生素的敏感性。 2方法　回顾分析 1999年 3月～ 2 0 0 0年 3月就诊于上海中山医院的患者痰标本中培养出的全部克雷伯杆菌 ,并采用微量稀释法测定 12种药物的最低抑菌浓度 ( MIC)。 3结果　共分离出克雷伯杆菌 197株 ,其中肺炎克雷伯杆菌 176株 ( 89.3% ) ,催产克雷伯杆菌 2 0株 ( 10 .2 % ) ,及鼻臭克雷伯杆菌 1株( 0 .5 % ) ,感染患者 90 %有明确的基础疾病 ,药敏测定结果为克雷伯杆菌对各种抗生素 (除泰能 )耐药率较高。敏感性最好的前 3位药为泰能、阿米卡星及头孢他啶。4结论　下呼吸道感染的克雷伯杆菌对常用抗生素耐药率偏高 ,提示进一步的微生物学检查 ,如广谱β-内酰胺酶 ( ESBL)To study the types of klebsiella bacilli of low respiratory tract infection and to investigate the drugs susceptibility of klebsiella bacilli.Method The 197 klebsiella bacilli strains were gotten from the patients' sputum culture.Determining the minimum inhibitory concentration (MIC) against klebsiella bacilli strains by micro dilution test.Results Klebsiella pneumonia 176 strains(89.3%),klebsiella oxytoca 20 strains (10.2%),klebsiella ozaenae 1 strain (0.5%) were isolated from the patients who had certain underlying diseases in 90 percent.The antimicrobial tests in vitro showed high resistant to all of the antibiotics (except imipenem),The susceptibility to imipenem?amikacin?ceftazidime more than others.Conclusions The klebsiella bacilli causing low respiratory tract infection showed high resistant to antibiotics.Treatment according to antimicrobial sensitivity test and ESBL test was recommanded.
