摘 要:目的 :了解超广谱β-内酰胺酶(ESBLs)的革兰氏阴性杆菌的检出率及耐药情况。方法 :对162株革兰氏阴性杆菌 ,采用双纸片法检测超广谱β-内酰胺酶(ESBLs) ,并比较每一种革兰氏阴性杆菌对13种抗生素的耐药性。结果 :共检出产ESBLs革兰氏阴性杆菌19株 ,检出率11.73%(19/162) ,162株中17株为克雷伯菌 ,此17株中产ESBLs菌4株占23.53% ,同样 ,大肠埃希菌占25.00%(2/8) ,肠杆菌属13.73 %(7/51) ,其他阴性杆菌6.98%(6/86)。比较产ESBLs的菌与不产ESBLs的菌对亚胺培南的体外抗菌活性最好 ,耐药率为00.0% ,其次为环丙沙星耐药率26.3% ,对氨卞西林、磺胺类均高度耐药(78.9 %~100 %) ,其余产ESBLs菌对9种抗生素的耐药率有显著增高(P<0.05)。结论 :产ESBLs的细菌 ,具有较高的交叉耐药性和多重耐药性 ,通过耐药质粒可传递给其他细菌 。Objective:To investigate the detective rates of those gramˉnegative bacilli strains capable of producting exˉtendedˉspectrumβˉlactamase(ESBLs)and to study their strength of antibiotic resistance.Methods:162strains of gram,we used doubleˉdisk synergy test method for detecting those gramˉnegative bacilli capable of producing ESBLs.The strength of antibiotic resistance to13different kind of antibiotics of each detected strain was measured and compared with each other between these detected strains of gramˉnegative bacilli,either ESBLs Lˉproducing or nonˉESBLs Lˉproducing.Results:Among162strains of gramˉnegative bacilli isolated from lab specimens,19strains of gramˉnegative bacilli capable of producing ESBLs were detected,the detective rate was11.73%(19/162).17strains belonged to klebsiella pneumoniase group,and4of these17strains(23.53%),were ESBLs Lˉproducing bacilli;8strains belonged to E.coli group,2of them(25.00%)were ESBLs Lˉproducing bacilli;51strains belonged to Enterobacteriacci,7of them(13.73%)were ESBLs Lˉproducing bacilli;6strains among those reˉmainder86strains of other groups(6.98%)were ESBLs Lˉproducing bacilli,either ESBLs Lˉproducing bacilli or nonˉESBLs Lˉproducing strains were as follows:the lowest antibioticˉresistance rate was0%for imipenum(IPM);the next lowest antibiotˉicˉresistance rate was26.3%for ciprofloxacin(CIP);there were high antibioticˉresistance rates for ampicillin(AM)and sinomin(SMZ).All of those ESBLs Lˉproducing strains of gramˉnegative bacilli showed remarkably high antibioticˉresistance rates(78.9%~100%)against9different kinds of antibiotics,(P<0.05).Conclusion:ESBLs producing strains cross or mulˉtiˉcross resistance were high among the commonly used antibiotice.Infectivn of resistant plasmid between the microorganism might result in local outbreak.
