摘  要:目的 探讨胸骨后巨大甲状腺包块的压迫作用对循环系统的影响,以及相关的临床问题。方法 手术治疗6例无继发甲亢的胸骨后巨大甲状腺包块,观察病人术前、术后的血压、心律及体力活动能力的变化,分析它与胸骨后巨大甲状腺包块的压迫作用的关系。结果 6例胸骨后巨大甲状腺包块并发的短期的高血压(6例),心律失常(4例),劳力性呼吸困难心悸(6例),术后均有明显好转。结论 胸骨后巨大甲状腺包块对颈胸部大血管的压迫,可导致高血压,心律失常和劳力性心悸,不应将这些并发症视为手术禁忌症,只有手术解除包块的压迫,才能根本治愈循环系统的并发症。Objective To investigate the compressive effect of giant substernal thyroid mass on cervicothoracic large blood vessels. Methods Changes of blood pressure, cardiac rhythm and exertion condition before and after operation were observed in 6 cases of substemal thyroid mass with no secondary hyperthyroidism and their relationship with compression of the mass were analysed. Results The short-term complications by the-mass as hypertension (6 cases), cardiac arrhythmias (4 cases) and exertion dyspnea (6 cases) were all improved after operations. Conclusion The compression of giant substemal thyroid mass on cervicothoracic large blood vessels may cause hypertension, cardiac arrhythmias and exertion dyspnea. These complications should not be regard as operative contraindicaton. By operative removal mass all these complications may be completely cured.
