超声普查武警战士精索静脉曲张的价值

摘　　要：目的　评价超声检查对武警战士精索静脉曲张的应用价值 ,探讨武警战士精索静脉曲张影响因素及其预防措施。方法　对新入伍的 15 69名武警战士的两侧精索静脉进行超声普查 ,发现精索静脉内径 >0 .2 0cm者进行彩色多普勒 (CDFI)检查 ,并测量其睾丸大小。入伍 1a后复查。结果　 15 69名战士中发现左侧精索静脉内径 >0 .2 0cm者2 2 8例 ( 14 .5 3 % ) ,其中CDFI发现静脉内不同程度返流者 163例 ( 10 .3 9% )均无明显临床症状 ,右侧精索内径均 <0 .2 0cm。1a后复查 ,左侧精索静脉内径 >0 .2 0cm者 3 79例 ( 2 4 .16% ) ,静脉内不同程度返流者 2 0 3例 ( 12 .94 % ) ,出现临床症状 84例 ,手术治疗 68例。结论　超声普查结合CDFI对武警战士精索静脉曲张诊断和随访有着重要的价值 ,长时间站立军事训练是影响武警战士精索静脉曲张的重要因素 。Objective To evaluate the ultrasonographic examination in mass screening of varicoceles in Armed Police forces soldiers,and to study the cause and the prevention of varicoceles in them.Methods Bilateral spermatic funiculus veins of 1569 recruits were examined by ultrasonography.Colour Doppler flow imaging(CDFI) was performed and the sizes of tests were measured on those whose spermatic funiculus veins were 0.20 cm or above in diameter.All subjects were followed up one year after.Results Left spermatic funiculus veins were 0.20 cm or above in diameter in 228 soldiers by CDFI, meanwhile the right spermatic funiculus veins of the 1569 soldiers were under 0.20 cm in diameter,and all of them had no related clinical symptoms.The results of follow-up showed that the left spermatic funiculus veins were 0.20 cm or above in diameter in 379 cases (24.16%) ,intravenous regurgitation was found in 203 cases (12.94%);84 cases manifested some related clinical symptoms,and 68 cases received operative treatment.Conclusions Combined with CDFI,ultrasonography plays an important part in the diagnosis and the follow-up of varicoceles in armed police soldiers.Standing sentry for a long time is an important cause of varicoceles.The prevention of varicoceles in armed police soldiers should be employed.
