胆汁泡蛋白在胆石症人群的含量分布及其临床意义

摘　　要：目的探讨胆汁泡蛋白在胆石症人群的含量分布,并观察利胆药物对其活性的影响。方法将纯化的泡蛋白免疫新西兰大白兔获得高效价抗体,建立 ELISA 标准曲线;并应用 ELISA 法测定正常人、胆石症病人胆汁和血清中泡蛋白含量同时观察不同溶石防石药物如利胆冲剂、胆酸钠等对泡蛋白的影响。结果建立了 ELISA 标准曲线,其曲线方程为 Y=0.035X(r=0.99);正常人胆汁和血清中 Mr 33 500泡蛋白含量都明显较胆固醇结石病人低[(0.007±0.003)g/L vs(0.050±0.011)g/L,(0.015±0.010)g/L vs(0.045±0.021)g/L,P<0.05];利胆冲剂治疗2周后,胆汁和血清泡蛋白含量显著降低,而对照组和胆酸钠治疗组未见明显变化。结论在不同人群胆汁泡蛋白的含量差异显著;利胆冲剂等溶石防石药物能降低胆汁和血清 Mr 33 500泡蛋白含量。Objective To investigate the distribution of Mr 33 500 vesicular protein in gallstone patients and effects of cholagogue dry syrups on its pro-nucleating activity.Methods Enzyme linked immuno-sorbent assay(ELISA)kits were developed by immunizing rabbits.The concentration of Mr 33 500 vesicular protein in serum and bile were compared between gallstone patients and controls.Fur- thermore,the change in Mr 33 500 vesicular protein was observed after cholagogue dry syrups and so- dium cholate treatment.Results One-step ELISA equation was Y=0.035X (r=0.99).The vesicular protein concentrations in serum and bile of cholesterol gallstone group[(0.45±0.021)g/L and(0. 050±0.010)g/L]were significantly higher than those in the pigment stone group[(0.017±0.011) g/L and(0.014±0.009)g/L]and controls[(0.015±0.010)g/L and(0.007±0.003)g/L](P< 0.05).After administration of cholagogue dry syrups for 2 weeks,the concentrations of Mr 33 500 vesicular protein in the serum and bile were significantly decreased in gallstone patients.However,the same results were not achieved in sodium cholate and control groups.Conclusions The concentrations of Mr 33 500 vesicular protein are different in different populations.Cholagogue dry syrups can de- crease biliary lithogenecity by reducing the pro-nucleating protein.
