摘　　要：目的 :观察骨髓增生异常综合征 (MDS)骨髓塑料包埋切片中原位细胞凋亡状况。方法 :用 DNA末端原标记 (ISEL)方法检测 33例 MDS患者骨髓塑料冷包埋切片中凋亡细胞 ,缺铁性贫血 15例作对照 ,并对 MDS之 ISEL 阳性细胞经常规染色光学显像镜分析细胞来源。结果 :33例 MDS的凋亡细胞平均为 (38.0 6± 2 4.6 1)个 /m m2 ,对照组为 (19.2± 7.98)个 /m m2 ,P <0 .0 1,差异有极显著性意义。 MDS中低危组 (RA+RAS)凋亡率(43.5 8± 39.40 )个 /mm2 较高危组 (RAEB+RAEB- T) (2 6 .0 4± 2 1.0 2 )个 /mm2 高 (P <0 .0 1) ,差异有极显著性意义。ISEL 阳性细胞主要来源于粒、红、巨三系造血细胞 ;分别占 38.45 % ,2 7.33% ,7.87%。结论 :MDS存在过度凋亡 ;MDS之 RA+RAS组凋亡高于 RAEB+RAEB- T组 ;凋亡细胞主要来源于三系造血细胞。Objective:To verify the in situ apoptotic cell situation on bone marrow biopsy section in patients with myelodysplastic syndrome (MDS).Method:Apoptosis cell were detected in 33 MDS cases by DNA in situ end labelling (ISEL) with 15 cases of iron deficiency anemias (IDA) as control.Apoptotic cells source were defined by morphologic analysis of ISEL positive cells under light microscopy.Result:Mean apoptotic cell number in MDS group is 38.06 ± 24.61 /mm 2,with a mean apoptotic number of 19.20 ± 7.98 /mm 2 in control group (P< 0.01 ).RA/RAS subtype of MDS had a higher apoptosis ratio ( 43.58 ± 39.40 /mm 2) than that in RAEB/RAEB T subtype group ( 26.04 ± 21.02 )(P< 0.01 ).Apoptosis in granulocyte,erythroid cell and megakaryocyte occupied 38.45 %, 27.33 %, 7.87 % of total apoptotic cells respectively.Conclusion:Overapoptosis is existed in MDS;RA/RAS group had a higher apoptosis ratio than RAEB/RAEB T group;Apoptotic cells derive mainly from granulocytic,erythroid and megakaryocytic hematopoietic lines.
