硝酸甘油介入心肌显像分析程序综合应用的诊断价值

摘　　要：目的　探讨心肌 SPECT的分析程序结合硝酸甘油 (NTG)介入对检测存活心肌的应用价值。　方法　对 33例冠心病患者 (CAD)及 2 0例对照者进行 99m Tc- MIBI心肌显像及 NTG介入显像。 11例 CAD患者行冠状动脉搭桥术 (CABG)后复查静息心肌显像。分别应用常规分析、三维心肌立体显像、圆周剖面曲线、多感兴趣区放射性计数、极坐标靶心图等进行综合诊断。　结果　 NTG介入后 CAD患者灌注减低区的填充面积均值为 14.19%。NTG介入前后的灌注面积有显著性差异 (P<0 .0 0 5 )。综合分析法检出的存活心肌节段明显高于常规分析 (P<0 .0 5 )。以 CABG术后显像为对照标准 ,NTG介入综合判断活力心肌的阳性预测值为 85 .7%。　结论　运用多种指标综合分析Objective\ To observe the diagnostic value of nitroglycerin administration myocardial SPECT in detecting myocardial viability with quantitative analysis.\ Methods\ \+\{99m\}Tc\|MIBI myocardial SPECT and nitroglycerin(NTG) administration were performed in 33 cases of coronary artery disease(CAD) and 20 normal persons.\ Of the patients,11 were performed CABG and underwent rest \+\{99m\}Tc\|MIBI SPECT after operations.\ A comprehensive analysis was used by routine method, stereophotogrammetry of myocardium,circumferential profiles, counts of multiple ROIs and Bull's\|eye analysis.\ Results\ The mean defect areas were significantly decreased after NTG administration(P<0 005).\ The mean filling sizes were 14 19% according to circumferential profiles.\ The mean counts of the defect regions were significantly increased after NTG(P<0 05).\ Conclusion\ The administration of NTG may improve the detection of viability in hypoperfused regions, and the comprehensive analysis provided complementary information for detecting viable myocardium.\;
