选择性腰骶神经后根切断术治疗痉挛性脑瘫的并发症分析

摘　　要：目的 :观察和了解选择性腰骶神经后根切断术 (SPR)治疗痉挛性脑瘫后的反应和并发症。方法 :通过对 2 0 9例以下肢痉挛为主的脑瘫患者行 L2 ～ S1 SPR后的观察和随访 ,对术后不良反应和并发症作了分析和研究。 结果 :出现 9例 (4.3% )较为严重的并发症 ,包括支气管痉挛、骨盆倾斜并发症、癫发作和脊柱前突增加。其他并发症有高热、血尿、脑脊液漏、腹胀痛和腿乏力等。 结论 :SPR能有效地解除痉挛 ,但会带来许多并发症 ,应严格掌握手术适应证 ,采取积极有效的预防措施 ,才能减少和避免术后不良反应和并发症的发生。Objective:To investigate the complications after selective posterior rhizotomy(SPR) in the treatment of spastic cerebral palsy. Methods:Two hundred and nine patients with lower limb spastic cerebral palsy undergone L 2 S 1 SPR were included in this study, the postoperative complications were evaluated. Results:Severe complications including brochospasm, pelvic obliquity and epileptic attacks occurred in 9 patients (4.3%), mild complications included:hyperthermal, hematria,leakage of cerebrospinal fluid, abdominal pain and low limb weakness, etc . Conclusion:The effect of SPR is remarkable in relieving muscle spasm, at the same time there are a number of complications, therefore, the indication for SPR should be strictly controlled and measures should be carried out to reduce the complications.
