摘　　要：目的 :评价普乐林注射液对急性心肌梗死患者梗死面积和心肌缺血总负荷的影响。方法 :将 10 2例急性心肌梗死患者随机分为普乐林治疗组和对照组 ,记录每例患者入院时和入院第 11日的血压、心率、12导联心电图及 2 4小时动态心电图 ,用 Wagner的 QRS记分法进行 QRS记分 ,预测梗死面积 ,计数有 Q波导联数(Qn)及 Q波总幅度 (ΣQ) ,计算 2 4小时心肌缺血总时间和缺血总负荷 ,并观察住院期间 (2～ 5周 )梗死后心绞痛及再闭塞的发生率。结果 :普乐林注射液治疗 11日后 ,降低心肌缺血总时间和缺血总负荷的幅度均较对照组增加 ,有 Q波导联数减少 ,Q波总幅度降低 ,梗死面积缩小 ,梗死后心绞痛的发生率较对照组低 ,再闭塞的发生率少于对照组。结论 :普乐林注射液有改善急性心肌梗死患者的心肌缺血状态。Objective:To determine the effects of Puerarin injection(普乐林注射液) on the infarct size and total ischemic burden in acute myocardial infarction patients.Methods:A singleblind randomized clinical trail was conducted in 102 patients with acute myocardial infarction who were given either Puerarin injection or not.The blood pressure,heart rate,electrocardiogram and Holter monitoring were recorded before therapy and 11 days after completion of the Puerarin injection therapy.The size of infarct with the method of Wagneris QRS scoring system was estimated and the leads with Q wave and the amplitude of Q wave was counted.The total ischemic time and burden were counted.The incidence of angina postinfarction and reinfarction in hospital were also observed.Results:After Puerarin injection therapy,the leads with Q wave and the total amplitude were decreased ,the infarct size and total ischemic burden were lower,the attacks of angina and reinfarction before discharge were also fewer than the controls.Conclusions:Puerarin injection therapy may improve myocardial ischemia after acute myocardial infarction,and prevent the extension of infarction size.
