摘 要:目的:探讨甘氨酸对急性坏死性胰腺炎 (ANP)的治疗作用及其对肿瘤坏死因子和内毒素含量的影响 ,并观察 TNF- α和内毒素 (Endotoxin,ET)在 ANP病程中的动态变化。方法 :标准 Wistar大鼠 10 5只随机分为正常对照组、ANP组及治疗组 ,采用制作十二指肠闭袢的方法复制大鼠 ANP模型 ,动态观察 TNF- α和 ET含量变化 ,同时观察各组大鼠不同时间胰腺组织的病理变化、大鼠 2 4h病死率及平均生存时间。 结果:甘氨酸可明显降低 ANP大鼠血 TNF- α含量 ,但对 ET无明显影响。还可降低 ANP大鼠 2 4h病死率及延长生存时间。结论:甘氨酸可以通过明显降低 TNF- α含量显著改善 ANP大鼠的预后。Objective: The purpose of this experiment was to evaluate the therapeutic effects of glycine on necrotic pancreatitis (ANP) in rats and determine the contribution of TNF-α and endotoxin (ET) in the progression of ANP. The influence of glycine on the degree of TNF-α and ET was further investigated. Methods: One hundred and five Wistar rats were randomly divided into three groups as normal control, ANP and ANP with glycine therapy. Each group had thirty-five rats. Twenty of them were taken for the blood samples collection and the other fifteen for observation of twenty four hour fatality and average survival time. The ANP model was made by following Chetty method (made a closed duodenal loop and injection of 0.3 ml infective human bile into the loop). The blood samples were colleted from veins at three and eight hours after the model was made. Meanwhile, we also observed the change of pancreatic morphology. Result: The levels of TNF-α and ET were more obviously elevated in ANP rats. Glycine could radically reduce the degree of TNF-α in ANP rats, but had no obvious influence on the degree of ET. It could also reduce the twenty-four hour fatality and prolong the average survival time in ANP rats. Conclusions: TNF-α and ET play an important role in ANP. Glycine may improve the prognosis of ANP rats by reducing TNF-α radically. The mechanism may be that glycine may open chloride channels in Kupffer cells and accelerate influx of chloride and the increased chloride influx most likely leads to inactivation of Ca~ 2+ channels of Kupffer cells by hyperpolarizing the cell membrane. At last, the reduced Ca~ 2+ in Kupffer cells resulted in the decreasing of the level of TNF-α
