江苏省世行贷款血防项目疾病控制效果评价

摘　　要：目的　评价江苏省世行贷款血防项目疾病控制效果。方法　按照世行贷款血防项目策略 ,于 1992～ 1998年在全省项目地区实施防治规划 ,并同步进行纵向观察 ,对人、畜血吸虫病感染率、钉螺自然感染率、阳性钉螺密度等指标的变化进行分析和评价。结果　实施项目后 ,流行范围大幅压缩 ,34个项目县 (市、区 )中 ,先后有 16个达到了血吸虫病传播阻断标准 ,4个已达血吸虫病传播控制标准 ,尚有 14个未控制流行 ;项目地区流行人口减少了 5 9.85 % ;人群感染率下降 48.10 % ,家畜(牛 )感染率下降 5 1.85 % ;而易感地带面积、活螺密度、阳性螺密度、钉螺感染率均明显上升 ,特别是阳性螺密度和钉螺感染率分别上升了 111.0 8%和 93.71% ;沿江地区血吸虫病疫情明显较内陆、山区严重。结论　世行贷款血防项目实施后虽然取得了显著成效 ,但沿江地区尚未能有效阻断血吸虫病传播。项目结束后 ,如果防治措施有所放松 ,将会出现疫情反弹 ,应予高度重视。Objective To evaluate the effect of the disease control in World Bank Loan Project for Schistosomiasis Control in Jiangsu Province. Methods The control approaches were carried out according to the control strategy of World Bank Loan Project for Schistosomiasis Control from 1992-1998 in Jiangsu Province. Some indicators of the epidemic situation were surveyed, including the prevalence in human population, prevalence in cattle, snail area, natural snail infection rate, and density of infected snails. Results After intensive control work through the World Bank Loan Project, the endemic areas have reduced remarkably. Among the 34 project counties, 16 have reached the criteria of transmission interruption for schistosomiasis, 4 have reached the criteria of transmission control and 14 were still endemic by the end of 1998. The population of the project epidemic area has reduced by 59 85%. The prevalence rate in human and cattle decreased by 48 10% and 51 85% from 1992 to 1998 respectively. However, the areas with high transmission potential, density of snails, density of infected snails and snail infection rate increased by the end of 1998. The density of infected snails and snail infection rate increased by 111 08% and 93 71% respectively. The degree of endemicity for schistosomiasis in the regions along the Yangtze River was more serious than that in the plain and hilly regions. Conclusion Although the effectiveness of the World Bank Loan Project for Schistosomiasis Control is remarkable, the schistosomiasis transmission has not been controlled in regions along the Yangtze River. If the control approaches cannot continue, it is possible for the resumption of transmission of schistosomiasis.
