摘 要:目的 了解食物过敏在婴幼儿哮喘中所占的比例。方法 对 15 8例婴幼儿哮喘患儿血清中食物特异性IgE(sIgE)进行检查。 结果 43 0 %的患儿血清中存在着食物sIgE ;sIgE的血清浓度在 3级 (3 5ku/L)以上的患儿 ,鸡蛋白为 8 1%、牛奶 8 1%、花生 5 4%、小麦 4 0 %及黄豆 1 3 % ;患儿中牛羊肉sIgE阳性率很低。仅有 2例患儿有明确的进食食物后 ,引起喘息发作的病史 ,其余则临床表现为哮喘伴湿疹、荨麻疹及过敏性鼻炎。对其中 130例患儿同时进行的吸入过敏原筛查试验 (Phadiatop)中 ,Phadiatop阳性为 44例 (33 8% ) ,而混合食物抗原 (fx5E)阳性的为 5 8例 (44 6 % ) (P <0 0 5 )。结论 在婴幼儿哮喘患儿中食物IgE阳性率比吸入物IgE阳性率更高。Objective To realize the proportion of food allergy in infant asthma. Method 158 tests of food specific serum IgE(sIgE)were performed on patients with infant asthma.Results 43 0% of the patients have been found to have food specific sIgE.Among all the cases we tested,the percentages of high level of (>grade 3,3 5ku/L)sIgE specific to various food as egg white,milk,peanut,wheat and soy bean were 8 1%,8 1%,5 4%,4 0%,and 1 3% respectively.The positive sIgE values to beef and mutton were low.In the clinic,only two patients in the group could provide the definite case history of inducing asthma after taking food;most other patients had complications as eczema,allergic urticaria or rhinitis.At the same time,Pharmacia CAP Phadiatop inhalation test was performed on 130 patients of the same group with infant asthma.44(33 8%) patients were allergic to the inhaled allergen,44 6%(58) of the patients were allergic to the food allergen(P<0 05).Conclusion It suggests that the positive rate of food sIgE is higher than that of inhalation sIgE in infant asthma patients' serum.
