摘 要:目的 :观察高频部分液体通气治疗吸入性肺损伤的效果 ,并与单纯高频喷射通气的疗效比较。方法 :将 16只犬经蒸气吸入造成吸入性肺损伤模型 ,并随机分为对照组和治疗组。 2组动物致伤后均行高频喷射通气 ,治疗组动物同时经气管导管将氟碳液体 ( 3ml/ kg)缓慢注入肺内 ,实行高频部分液体通气治疗 ,于 30、6 0及90分钟测定动脉血气、肺顺应性和气道阻力。结果 :治疗组动脉血氧分压 ( Pa O2 )进行性上升 ,各时间点与致伤后比较均有显著性差异 ( P均 <0 .0 5 ) ,动脉血二氧化碳分压 ( Pa CO2 )也逐渐增高 ,于 6 0和 90分钟显著高于致伤后水平 ( P均 <0 .0 5 ) ;与对照组比较 ,治疗组各时间点的 Pa O2 稍有升高 ,而 Pa CO2 于 90分钟显著增高 ( P<0 .0 5 )。在各时间点的动、静态气道阻力和动、静态肺顺应性 ,2组比较均无明显差异。结论 :高频部分液体通气与单纯高频喷射通气相比 ,更有利于动脉血氧合 ,但也有轻度 CO2 潴留现象。Objective:To observe the treatment effects of high frequency partial liquid ventilation on inhalation injury to compare with high frequency jet ventilation alone.Methods:Sixteen dogs with steam inhalation injury were randomly divided into control group and treatment group.Both groups were given high frequency jet ventilation after inhalation injury,while in the treatment group perfluorocarbon liquid (3 ml/kg) was slowly injected into lungs at the same time to establish high frequency partial liquid ventilation.Blood gas,lung compliance and airway resistance were measured at 30,60 and 90 minutes post ventilation.Results:Partial pressure of oxygen in artery(PaO 2) in treatment group at various intervals were higher than that in the control group (all P <0 05),meanwhile partial pressure of carbon dioxide in artery(PaCO 2) also increased at 60 and 90 minutes (both P <0 05).Compared with control group,PaO 2 in treatment group was slightly elevated ( P >0 05),but PaCO 2 in treatment group was remarkably increased at 90 minutes ( P <0 05).There were no significant differences in dynamic/static lung compliance and dynamic/static airway resistance between treatment group and control group.Conclusions:Oxygenation with high frequency partial liquid ventilation is better than that with high frequency jet ventilation alone,however the former also has disadvantage of retention of CO 2.
