产妇尿碘与新生儿甲状腺激素水平的相关性研究

摘　　要：目的　探讨产妇尿碘水平与其新生儿甲状腺激素水平之间的关系。方法　应用恒温消解测定法检测了 10 0例产妇 (研究组 40例 ,对照组 6 0例 )临产时尿碘水平。同时应用免疫放射分析 (IRMA)技术检测了两组新生儿脐血促甲状腺激素 (TSH)水平和总甲状腺素 (TT4)水平。结果　 1研究组产妇尿碘水平中位数值为 12 9.44μg/ L ,对照组为 2 5 8.2 6μg/L,两组间比较差异显著 (P<0 .0 1)。 2研究组新生儿脐血 TSH水平的均值为 (6 .0 5± 2 .89) m U/ L,对照组为 (5 .12± 2 .74)m U/ L,两组间比较差异显著 (P<0 .0 1)。研究组新生儿 TT4水平的均值为 (5 6 .48± 14.0 3) ng/ ml,对照组为 (73.13±12 .5 1) ng/ ml,两组间比较差异显著 (P<0 .0 1)。 3两组孕妇临产后尿碘水平和其新生儿脐血 TSH水平之间呈显著负相关(P<0 .0 5 ) ,和新生儿脐血 TT4水平之间呈显著正相关 (P<0 .0 1)。结论　产妇碘营养状况与其新生儿甲状腺素水平密切相关。Objective The correlation between urinary io dine levels in pregnant wo men and thyroid hormones of their neonatal umbilical cord blood was explord.Methods The urinary iodine levels of 100 pregnant women(40 form iod ine deficient ar eas,60 form iodine sufficient areas as control group) by normal birth were teste d by Acid-digestion Thermostatic Assay.With IRMA technique,the thyrotropin (TSH ) levels and total thyroid hormone (TT 4) levels of 100 newborns were tested re spectively.Results ①The median value of urinary iodine levels in study group w as significantly lower than that of control group (129.44 VS258.26μg/L,P<0.01).②The mean TSH levels of newborns in study group was signific antly higher than that in control group [(6.05±2.89)mU/L VS (5.12±2.74) mU/L ,P<0.01].The mean TT 4 levels of newborns in study group was significantly lower than that in control group[(56.48±14.03) VS(73.31±12.51) .ng/ml,P<0.01].③The urinary iodine levels in pregnant women w ere negatively correlated with the cord blood THS levels of their newborns in bo th group(P<0.05).The urinary iodine levels in pregnant women we re positively correlated with the cord blood TT 4 levels of their newborns in b oth groups(P<0.01).Conclusion The data emphasi ze the iodine nu trition status in pregnant women is closely related with the thyroid hormone of their newborns.
