去带盲结肠可控性膀胱术18例报告

摘　　要：目的　介绍去带盲结肠可控性膀胱术的临床经验。　方法　对 18例膀胱癌患者根治性全膀胱切除后 ,施行了去带可控性盲结肠膀胱术。　结果　获访 17例 ,平均随访 16 (6～ 2 8)个月。 16例能完全控制排尿 ,1例夜间轻微溢尿。插管均容易。术后 12月膀胱容量平均 415ml。充盈压力平均 2 2cmH2 O。无输尿管狭窄及反流。肾功能、血生化、尿液化验正常。　结论　去带盲结肠可控性膀胱术具有手术简便、容量大、内压低、无反流、可控排尿。Objective To reommend the clinical experience of a urinary reservoir. Methods A detenia cecocolon continent urinary reservoir was constructed for 18 patients after total cystectomy and the patients were followed up for a mean of 16 months. Results 16 patients had continent urination day and night without leakage. The average capacity of the urinary reservoir was 415 ml and the average intrareservoir pressure during filling was 22 cmH 2O after 12 months of operation. There was no evidence of ureteral reflux. Renal function and other laboratory findings were normal. Conclusion Detenia cecocolic bladder provides large capacity, low intrareservoir pressure, continent urination with low complication rate thus improving the life quality of the patients.
