摘 要:目的 :慢性肾衰竭 (CRF)尿毒症 (Uremia)患者临床多表现为肾亏血虚证 ,本实验探讨肾亏髓空的发病机制。方法 :将CRF患者血液透析超滤液 (UF)浓缩 ,凝胶层析分离出分子量 (MW)在 10 0 0~ 5 0 0 0daltons之间的中分子物质 (MMS)的三个不同组分 ,各制成两种浓度 ,加入到骨髓红系细胞培养体系中 ,分别于培养的三个不同时间加入3 1H-TdR 0 .7μci/ml,用显微摄像术和放射性同位素自显影技术观察红系细胞的结构 ,分类计数及标记指数 (LI)的动态变化。结果 :与正常组相比 ,MMS显著地抑制红系细胞的生长、成熟过程 ,并且不同的浓度之间有显著性差异 ;浓度越高 ,其抑制作用越大。结论 :MMSMW在 10 0 0~ 40 0 0daltons之间的组分是CRF患者血中的红细胞生长抑制因子 (EIF)。本实验结果从病理角度探讨了祖国医学“肾主骨生髓”、“生精化血”“精血同源”、“肾亏髓空”的发病机制 ,为探索清除CRF患者血中MMS的有效药物 ,寻求治疗肾性贫血的有效方法提供实验依据。Objective:To study the theory of 'renal asthenia-marrow hollow'in traditional Chinese medicines.Methods:Middle molecular weight substances with MW of 1000-5000 daltons were isolated from ultrafiltration of hemodialysis by chromatography.Erythropoietic bone marrow cells was isolated from normal subjects and cultured with two different concentrations of three MMS fractions. 3H-TdR was used to observe changes in cell structure and the index (LI) of 3H-TdR was measured by microphotography and autoradiography.Results:compared with the normal group,the second and the third fractions of MMS significantly inhibited the maturation and growth of erythropoiesis in a dose-dependent manner.These MWS had a molecular weight of 1000-4000 daltons.Conclusion:MMS is an erythropoietic inhibitoroy factors(EIF)in the blood of crf patients,contributing to the theory of 'kidney controlling bones','kidney producing bone marrow and bolld','Essence and Blood being derived from the same source',and of the 'kidney asthenia-marrow hollow'.
