摘  要:目的 探讨高血压病患者左室心肌肥厚定量过程的偏差。方法 用二维超声经胸测量 10 0例高血压病患者心肌 2 0个节段的厚度 ,与 30例正常者对照。结果 与正常组比较 ,高血压病组左室心肌厚度的不均匀分布比例明显高于对照组 (分别为 0 .5 5和 0 .0 5 ,χ2(校正 ) =5 .482 ,P <0 .0 2 5 ) ;常规测量左室心肌质量的方法受肥厚分布不均匀的影响 ,与心肌平均厚度之间差异显著 [分别为 ( 174.7± 74.7) g和 ( 2 0 6 .9± 83.8) g ,t =2 .86 8,P <0 .0 1]。结论 高血压病左室肥厚的分布和正常人比较是有差异的 ,应避免仅在与肥厚心肌均值偏差较大的部位采样、测量和计算。Objective To take a view of the distributing rules of hypertrophic myocardium in hypertension,and analyzing the decination in quantifying myocardial thickness.Methods There were 100 cases of hypertension that received echocardiographic scan for measuring each of the segmental thickness at left ventricle and comparing them with 30 cases of normal group.Results Comparing with normal group,the hypertrophic muscular distribution in hypertension group was,statistically,more uneven than that in contract group( 0.55 and 0.05,respectively,χ 2 (corrected)= 5.482,P< 0.025).The way of conventional measure of left ventricular mass (LVM) was affected by the nonuniform distribution in which there were different thickness at different regions,and comparing those calculating values of LVM with that of the average thickness,the differences were remarkable [( 174.7± 74.7)g vs ( 206.9± 83.8)g,t= 2.868,P< 0.01].Conclusions The hypertrophic distribution of left ventricle is different between hypertension and normal groups.Avoid selecting those regions in which average thickness is low statistically,for sample,measurement and calculation.
