诱发排卵治疗女性不孕症成功妊娠133例分析

摘　　要：目的 :总结我院诱发排卵为主治疗女性不孕症 133例的成功经验。方法 :回顾分析 1995年 4月至 2 0 0 0年 3月采用诱发排卵为主治疗女性不孕症成功妊娠 133例的不孕原因、治疗方法和妊娠结局。结果 :女性不孕症的不孕原因以排卵障碍为主 ,共 93例 ,占 70 %。其次为盆腔炎、子宫内膜异位症和盆腔肿瘤。妊娠结局为 :足月产 10 7例 ,早产 3例 ,流产 2 0例 ,异位妊娠 3例 ;分别占 80 .4% ,2 .3% ,15 %及 2 .3% ,多胎 4例 ,占 3%。诱发排卵疗程与妊娠相关 ,克罗米芬/人绒毛膜促性腺激素 (CC/h CG)方案诱发排卵 ,1～ 3周期治疗成功者 5 6例 ,占 83.6 %。结论 :处理女性不孕症重在查清楚不孕原因 ,针对原因进行治疗可收到立竿见影之效。诱发排卵治疗是不孕症的主要治疗措施 ,需注意防止卵巢过度刺激征 (OHSS)、流产及多胎等并发症。Objectives:To Summarize the successful experience of 133 infertile women,treated by ovulation induction.Methods:One hundred and thirty three infertile women treated successfully by induction ovulation in our hospital from 1995 to 2000, were retrospectively analyzed on causes of infertility,methods of treatment and outcome of pregnancy.Results:The major cause of infertility is anovulation.There were ninety three infertile cases(70%) due to anovulation among all cases.The other causes were pelvic inflammatory disease,endometriosis and pelvic tumors.Pregnancy outcome includes107 term deliveries,3 premature deliveries,20 abortions and 3 ectopic pregnancies.The incidence were 80.4%,2.3%,15% and 2.3% respectively.There were 4 cases with multiple pregnancy,the incidence was 3%.Fifty six (83.6% ) women became pregnancy after 1～3 courses of ovulation induction by clomiphen/human chorionic gonadotropin(CC/hCG)regimem.Conclusions:The best way of management of female infertility was investigating real causes of infertility.Treatment of infertile women in accordonce with the causes could got instant results.Ovulation induction was the major way of treating female infertility.We had to prevent complications of ovarian hyperstimulation syndrome(OHSS)、abortion and multiple pregnancy.
