生血合剂治疗老年再生障碍性贫血的临床研究

摘　　要：为观察生血合剂治疗老年再生障碍性贫血 (简称再障 )的临床疗效 ,探讨疗效机理 ,6 2例患者随机分为治疗组 (32例 )和对照组 (30例 )。治疗组中脾肾阳虚型用生血Ⅰ号合剂、脾肾阴虚型用生血Ⅱ号合剂 ;对照组 30例用康力龙。疗程 3个月。结果 :治疗组基本治愈 6例、缓解 9例、明显进步 12例、无效 5例 ,总有效率 84.38% ,治愈缓解率 46 .88% ,明显优于对照组 (P <0 .0 5 ) ,且未见肝损等不良反应。治疗组患者症状改善 ,外周血象明显升高 ,T淋巴细胞亚群比例趋向正常 ,自然杀伤细胞活性 (NKa)增加 ,白介素 2 (IL - 2 )产生减少 ,骨髓增生程度明显好转 ,均有统计学意义 (P <0 .0 5 ,P <0 .0 1)。提示生血合剂治疗老年再障疗效良好 ,安全无毒 ,其疗效机制可能是通过调整免疫 ,改善骨髓微环境 ,促进造血功能的恢复来实现的。To observe the clinical efficacy of 'Blood-Producing Mixture' in the treatment of senile aplastic anemia,62 cases were randomized into two groups:treatment group with 32 cases treated by No.1 'Blood-Producing Mixture' for those of spleen-kidney yang-deficiency and No.2 'Blood-Producing Mixture' for those of spleen-kidney yin-deficiency,and control group with 30 cases treated by Stanozolol.Results showed basic cure in 6 cases,remission in 9 cases,marked improvement in 12 cases and failure in 5 cases,and the total effective rate was 84.38%,cure and remission rate 46.88% and no negative effects in treatment group,higher than those in control group ( P <0.05).In treatment group,the clinical symptoms improved,peripheral blood count increasing,subsets of T lymphocyte tending to normalize,the activity of natural kill cell increasing,IL-2 reducing,production of bone marrow improving,with striking differences.
