简易下腔静脉临时滤过器在治疗下肢深静脉血栓中的应用

摘　　要：目的 :探讨简易下腔静脉临时滤过器在治疗下肢深静脉血栓 (DVT)时的疗效。方法 :对 13例下肢DVT患者采用放置简易下腔静脉临时滤过器结合经健侧股动脉插管持续加压灌注尿激酶溶栓治疗。结果 :13个下腔静脉临时滤过器均成功放置于所有患者的下腔静脉 ,术后无 1例患者并发肺栓塞 (PE)及其他严重并发症 ;13例患者中 12例患者患肢血管血栓完全溶解 ;平均尿激酶用量为 5 40万U ;平均疗程为 4.5d。结论 :对DVT患者采用放置简易下腔静脉临时滤过器结合经健侧股动脉插管持续加压灌注尿激酶 ,可有效溶解血栓 ,防止患者并发肺栓塞 ,值得临床推广。Objective: To investigate the efficacy of self-made temporary IVC-filter on preventing pulmonary embolization(PE) in patients with deep vein thrombosis(DVT). Methods: 13 DVT patients receiving transarterial thrombolysis therapy were enrolled into this study. 13 self-made temporary IVC-filters were deployed into each patient's IVC. The patients' clinical symptoms and outcomes of Duplex ultrasound examination were compared respectively. Result: All self-made IVC-filters were deployed successfully into each DVT patient's IVC without any serious complications. The embolus was completely lysed in 12 of 13 patients. Average urokinase dosage was 5,400,000U and therapic time was 4.5 days. Conclusion: Self-made temporary IVC-filter proved to be efficient on preventing pulmonary embolization in DVT patients who received transarterial thrombolysis.
