甘肃省境内6376例火车创伤院前分析

摘　　要：目的　总结甘肃省境内 1 985年～ 1 999年间 ,火车创伤的发生概况、院前的流行病学特征。方法　统计火车创伤发生总量、分布规律。结果　甘肃省境内 1 985年～ 1 999年间共发生火车创伤 6 1 4 3件 ,涉及人员伤亡 6 376人。院前死亡 3 2 56人 ,伤 2 878人 ,轻微伤 2 4 2人。院前死亡率 51 .0 7% (3 2 56 6 376)。职业以农民 66 .34 % (42 30 6 376)和无业人员 1 3 .2 5 % (845 6376)最多。发生地点以区间 46 .39% (2 850 61 4 3件 )、站内 39.46 % (2 4 2 4 61 4 3)最多。认为铁路上行走 35.1 3 % (2 2 4 0 6376)、穿越站场 2 6 .99% (1 72 1 6 376) ,扒、钻、跳车 1 7.61 % (1 1 2 3 6376)等违章行为是导致火车创伤发生的主要原因。地域分布甘肃东部发生绝对数 (4880 )明显多于甘肃西部 (1 4 96)。结论　火车创伤属频发事故 ,平均 42 5人 年。院前死亡率高 ,如考虑院内死亡则总死亡率更高。应加强对高危人群的铁路法规教育 ,做好保护学生儿童的工作。Objective To investigate the general picture and the epidemiological prehospital character of railway traffic injuries(RTI)during 1985-1999 in Gansu province.?Methods The total number and the pattern of distribution of RTI were dealt statistically.?Results In all 6?143 cases of RTI occurred from 1985-1999 in Gansu province,the number of prehospital deaths were 3?256,severe injuries 2?878 and slightly wounded 242,among all 6?376 victims,the accidents mostly happened at the juncture of regions(2?850/6?143),and in stations(2?424/6?143).The high risk group consists of mainly the peasants 4?230/6?376 and unemployed(845/6?376).All kinds of violating traffic regulations,such as walking on the line(2?240/6?376),walking across the line (1?721/6?376) and breaking rules and regulations in getting on or off the train 1?123/6?376,respectively,were the main reasons leading to RTI.The actual wounded were 4?880 in the eastern part of Gansu while 1?496 were wounded in the western part of it.?Conclusion The accidents of RTI happened so frequently that average 425 persons per year were its victims with high death rate.The effective way to decrease RTI is to strengthen propaganda on railway traffic safety and to protect the students.It is very important to improve railway transport management and enhance civilians concept of traffic regulations.
