高血压伴和不伴心肌肥厚病人^(210)TI心肌灌注断层显像

摘　　要：目的评价201TI心肌灌注断层显像在高血压伴和不伴心肌肥厚病人中心肌灌注情况,并讨论其灌注异常的原因。方法对85例疑冠心病病人进行201TI心肌灌注断层显像,灌注异常者均在显像后1个月内行冠状动脉造影。结果在高血压伴心肌肥厚组(85.7%)中,灌注异常率较无高血压组(39.3%,P<0.01)和高血压无心肌肥厚组(61.2%,P<0.05)显著;半定量评分均较无高血压组(P<0.01)和高血压无心肌肥厚组(P<0.05)显著;灌注异常病人冠状动脉造影正常率(9/18)比高血压无心肌肥厚组(3/19,P<0.05)和无高血压组(0/13,P=0.044)显著。高血压无心肌肥厚组的半定量评分较无高血压组显著(P<0.05),灌注异常病人冠状动脉造影正常率与无高血压组无显著性差别(P>0.05)。结论高血压心肌肥厚病人201TI心肌灌注异常的发生率较高。其灌注异常不仅可能由冠脉大血管病变所致,而且还可能由冠脉小血管病变所致。Objective To evaluatethemyocardialperfusioninhypertensivepatientswithandwithoutleftventricularhypertrophy usingthallium-201( 201 TI)myocardialscintigraphyandexplorethemechanismthatcausestheperfusionabnormalities.Meth ods Thallium-201myocardialscintigraphywasperformedin85patientswithclinicallysuspectedcoronaryarterydiseaseto assess themyocardialperfusionstatus.Forpatientswithmyocardialperfusionabnormalitiesas indicatedby thescintigraphy,coro-naryangiographywas orderedwithin1month.Re sults Theincidenceof 201 TI myocardialperfusionabnormalitiesin hyper-tensivepatientswithventricularhypertrophy(85.7%)wassignificantlyhigher thanthatinthenormotensive(39.3%,P<0.01)andhypertensivepatientsnotcomplicatedwithhypertrophy(61.2%,P<0.05).Thesemiquantitativescorewashigherinthehyper-tensivepatientswithhypertrophythaninthenormotensive(P<0.01)andhypertensivepatientswithouthypertrophy(P<0.05).Morehypertensivepatientswith hypertrophy(9/18)hadnormalresultsof coronaryangiographythanthenormotensive(0/13,P=0.044)andhypertensivepatientswithouthypertrophydid(3/19,P<0.05).Con clu sions A higherrateof 201 TI perfusion abnormalities occursin thehypertensivepatientswith ventricularhypertrophy,whichmaybe attributedto thepathological changesinthemajorcoronary vesselsandto thecoronarymicrovasculardiseasesas well.
