彩色多普勒超声心动图测定左室舒张功能敏感指标的临床研究

摘　　要：目的 :筛选彩色多普勒超声心动图 (CDFI)检测左室舒张功能敏感而可靠的指标 ,探讨高血压各期、冠心病各亚型舒张功能变化规律 ,研究左室质量 (LVM)与舒张功能的关系。方法 :用CDFI对高血压组 ( 70例 )、冠心病组 ( 114例 )、健康人组 ( 70例 )左心结构、左室质量和舒张功能进行检测对比 ,并与放射核素心血管造影 (RNAG)做相关对照分析。结果 :①筛选出舒张早期峰值流速 (PVE)、舒张晚期峰值流速 (PVA)、A峰与E峰流速比 (A/E)。充盈指数 (SV/S)、主动脉瓣下舒张晚期峰值流速 (aVAo)、等容舒张时间 (IDT)六项指标检测舒张功能的敏感性、特异性均 >6 0 %。②高血压病、冠心病左室舒张功能指标较健康人有显著差异。其中 ,LVM、PVA、A/E、aVAo和IDT与高血压分期呈正相关 ,PVE和SV/S与高血压分期呈负相关。而冠心病各亚型之间舒张功能指标无显著差异。结论 :本研究中 ,CDFI测定左室舒张功能有六项较敏感和特异的指标。高血压、冠心病均存在LVDD。LVDD的程度与高血压分期有关 。Objective: ①To screen the sensitive and reliable indexes in evaluating left ventricular diastolic function(LVDF) by color doppler flow image(CDFI);②To observe the change of these indexes in the patients with hypertension and coronary heart disease(CHD);③To study the relation between left ventricular mass (LVM) and LVDF.Methods: All subjects are divided into three groups:hypertension group (n=70),CHD groups (n=114) and control group (n=86).Each subject's variables of left heart structure,LVM and LVDF were measured by CDFI.Those variables about LVDF were also measured by radionuclideangiography,the correlative analysis of these data was done.Results: ①There were six sensitive and reliable indexes:the early diastolic filling peak velocity (PVE),the late diastolic filling peak velocity (PVA),the ratio of late to early peak velocity (A/E),filling index(SV/S),late diastolic peak velocity under aorta valve(aVAo)and isovolumic diastolic time(IDT).Their sensitivity and specificity are more than 60%.②There existed significant difference in these indexes among hypertension,CHD and control group.LVM,PVA,A/E,aVAo and IDT were positively related to the phase of hypertension,while PVE and SV/S were negatively related to it.But the change of all these indexes didn't related to the types of CHD.Conclusion: There are six sensitive and reliable indexes in evaluating LVDF by CDFI.LVDD exists in hypertension or CHD.The degree of LVDD is positively related to the phase of hypertension,but isn't to the types of CHD.
