高血压及脑卒中红细胞镁/钠交换率与类洋地黄相关性研究

摘  要:目的 :探讨原发性高血压 (EH)、脑出血 (CH)和脑梗死 (CI)急、慢性期患者和正常体检者血浆镁、钠(PMC、PSC) ) ,以及红细胞钠、镁含量 (ESC、EMC)和钠镁交换率 ,以及与类洋地黄物质 (EDLS)相关性。方法 :采用原子吸收光谱火焰法和放射免疫法测定 2 3例EH(EH组 )、2 1例CH(CH组 )和 34例CI(CI组 )患者血浆EDLS浓度 ,并与 2 0例健康体检者 (对照组 )比较。结果 :①EH患者PMC、PSC与正常人无明显统计学意义 ,而EMC明显降低 (P <0 .0 5 ) ,红细胞钠依赖性镁外流最大速率 (VNDmax)、红细胞总镁外流最大速率 (VTmax)明显增高(P <0 .0 5 ) ,EMC、VNDmax与EDLS呈明显负相关。②CH和CI急慢性期比较 ,ESC明显增高 (P <0 .0 1~0 .0 0 1) ,而EMC则明显降低 (P <0 .0 1) ,VNDmax、VTmax明显增快 (P <0 .0 1~ 0 .0 0 1)。EMC、VNDmax与EDLS呈明显负相关 ,而与ESC呈明显正相关。结论 :细胞内外钠 镁转换率加快是造成细胞内钠高镁低的最主要原因 。Objective:To investigate the plasma sodium concentration (PSC), the plasma magnesium concentration (PMC), erythrocyte sodium content (ESC), erythrocyte magnesium content(EMC), erthrocyte Na +/ Mg 2+ exchange rate and its relationship with endogenous digitalis like substances (EDLS) in patients with essential hypertension(EH), cerebral hemorrhage(CH) and cerebral infarction(CI).Method:PSC, PMC, ESC, EMC and Na +/Mg 2+ exchange rate were measured by flameassay of atomic absorption spectrophotometry. EDLS was measured with radioimmunoassay.Result:①PSC, PMC and ESC in EH group was not significantly different from those in normal control. On the contrary, the EMC was lower (P< 0.05 ) and maximum rates of total magnesium(VTmax)and sodium dependent magnesium(Vdmax)were higher than those in controls(P< 0.05 ). Furthermore, the EMC and VNdmax were negativly correlated with EDLS.②When compared with controls, ESC, VTmax and VNdmax were significantly higher(P< 0.05 ~ 0.001 ) and EMC was markedly lower(P< 0.01 ~ 0.001 ) than that in both CH and CI group (P< 0.01 ~ 0.001 ). Meanwhile, the ESC was significantly positively correlated with EDLS.Whereas the EMC and VNdmax were significantly negatively correlated with EDLS in acute and chronic stage.Conclusion:Increased in EH and stroke patients, increase the internal exfernal cellular interchanging rate of Na Mg is the best important factor to cause hypernatremia and hypomagnesemia. EDLS takes part in and adjusts this processes.
