摘 要:目的 探讨离体猪心温血再灌注模型在离体心脏保存研究中的应用。方法 13头实验用猪 ,心脏停跳后取出猪心 ,低温保存后 ,利用体外循环机器及其管道建立离体猪心再灌注和做功模型。体外循环机升温至 37℃ ,主动脉逆灌复跳 ,维持灌注流量 30 0~ 40 0ml/min ,稳定 15min后 ,钳闭主动脉供血管 ,改为左心房顺灌 ,心脏后负荷为 10 0 12 0cmH2 O ( 1cmH2 O =0 .0 98kPa)。测定基础、复跳 15min、做功 15min的心脏左室舒张末压 (LVEDP) ;左室内压最大变化率(±dp/dt)。 结果 10例自动复跳 ,3例电击除颤复跳 ,复跳 15minLVEDP、±dp/dt分别为( 13.49± 2 0 .2 9)mmHg( 1mmHg =0 .133kPa)、( 70 11.92± 874.65 )mmHg·s-1,( -4 974.61±986.19)mmHg·s-1;做功 15min后 ,LVEDP、±dp/dt分别为 ,( 9.0 0± 19.68)mmHg·s-1、( 62 10 .38±5 65 .88)mmHg·s-1、( -4 832 .92± 941.44 )mmHg·s-1。其中 ,做功 15min的+dp/dtmax与基础比较 ,P =0 .0 0 5。结论 此离体猪心再灌注模型可用于大动物离体心脏保存的研究 ,方法可靠。Objective To introduce the application o f isolated pig heart warm blood reperfusion model in the study of isolated heart preservation.Methods First CPB was set up in 13 experimental pigs,the n the pig heart was taken out after heart arrest.By using the extracorporeal cir culation machine and duct to establish isolated pig heart reperfusion and workin g model.Heart was rebeated with perfused in aorta while the temperature of CPB c irculation was 37℃.Perfusing in the left atrium with clamping the aorta duct after keeping the flux of perfusion 300-400ml for 15min.The isolated hearts can well work when post-load maintained 100~120cmH_2O.LVEDP and ±dp/dt wer e recorded at the base line,15min after rebeating and 15 min after post-load maintained.Results Twn isolated hearts successfully rebeated and 3 h earts rebeated by defibrillation.LVEDP,±dp/dt were (13.49±20.29)mmHg,((7011.92±)874.65)mmHg·s^(-1),(-4974.61±986.19 15)mmHg·s~ (-1) 15min after rebeating and ((9.00±)19.68)mmHg,(6210.38±565.88 )mmHg·s^(-1),(-4832.92±941.44)mmHg·s^(-1) 15min after post -load maintained,respectively.+dp/dt at 15 min after post-load maintained sho wed significant difference as compared withbase line.Conclusion By using this model,various heart function in dexes can be determined conveniently.This isolated heart reperfusion model can b e used in the study of isolated heart preservation in large animals including pig.
