摘　　要：目的：研究减肥者服用西布曲明前后血清瘦素（Ｌｅｐｔｉｎ）、胰岛素（ＩＮＳ）、血脂（ＴＧ、ＴＣ、ＨＤＬ－Ｃ、ＬＤＬ－Ｃ）、谷丙转氨酶（ＡＬＴ）。谷草转氨酶（ＡＳＴ）、体重指数（ＢＭＩ）、腰臀围比（ＷＨＲ）、体重、心率、血压等的改变，评价该药的临床效果 方法：肥胖或超重患者共７８例，其中男２６例，女５２例，随机双盲分组，服用西布曲明２４周，Ｌｅｐｔｉｎ。ＩＮＳ用ＲＩＡ法测定，血脂用酶法测定，其它指标常规方法测定。结果：治疗后对照组与治疗组ＢＭＩ、体重、腰围比较有显著性差异（Ｐ＜０．０１）；治疗组治疗前后Ｌｅｐｔｉｎ，ＩＮＳ、血脂、体重、ＢＭＩ、心率、腰围、ＷＨＲ比较均有显著性差异（Ｐ＜０．０１），而血压、ＦＢＧ比较无显著性差异（Ｐ＞０．０５），治疗后Ｌｅｐｔｉｎ与 ＢＭＩ相关显著（ｒ＝０．６１１，Ｐ＜０．０１），腰围与ＩＮＳ相关显著（ｒ＝０．４０５，Ｐ＜０．０１），ＡＬＴ、ＡＳＴ有在正常范围的升高。结论：西布曲明减肥疗效显著，并对降低Ｌｅｐｔｉｎ、ＩＮＳ、血脂有一定效果。Object: Investigate the change of serum leptin , insulin(INS), fat of blood(TG, TC, HDL - C, LDL -C), ALT, AST. body mass index (BMI), WHR, weight, heart rate, blood pressure in the persons who reduce weight before and after taking Sibutramine. to evaluate the clinical effect of the drug. Method: 78 cases of obesity or overweight. 26 cases of male, 52 cases of female, grouped in random and double - blind, take Sibutramine for 24 weeks. Leptin and INS were assayed by RIA, fat of blood were assayed by emzyme method. Other indexs were determined by common method. Result: After treatment, compare to control. BMI. weight and WHR of therapeutical group have significant difference( P<0. 01). For therapeutical group. before and after treatment. Leptin. INS, fat of blood, weight. BMI. heart rate, waistline and WHR also have significant difference( P<0. 01). but blood pressure and FBG have no significant difference( P>0. 05). After treatment, correlation of leptin and BMI was significant. (r = 0. 611, P<0. 01). correlation of waistline and INS was also significant ( r = 0. 405, P<0. 01). ALT and AST increased within normal range. Conclusion: The effect of Sibutramine to reduce weight was significant. Sibutramine also has some effect on reducing leptin. INS and fat of blood, but liver ailment patients take cautiously.
