经吻合器治疗重度痔的临床应用价值(附36例报告)

摘　　要：目的　探讨肛门镜下吻合器痔切除术 (PPH )的临床应用价值。方法　分析重度内痔 (Ⅲ度、Ⅳ度内痔 )经吻合器切除的诊治资料。结果　经肛门镜下采用 33mm吻合器切除重度内痔 36例 ,平均手术时间 10min ,术后平均住院 3 5天 ,6例术后第 2 7天有便血 ,经保守治疗后好转。 12例术后当天无肛门疼痛。随访 15个月效果良好。结论　经肛门镜下吻合器痔切除术是一种新技术 ,具有安全、有效、住院时间短和恢复快的优点 。Objective To evaluate the efficacy of the stapled prolapsectomy(PPH) in treatment of third or fourth degree hemorrhoids.Methods Analyze the date of the third or fourth degree hemorrhoids operated by PPH.Results Total of a 33mm circular staplar,mean surgical time was 10 minutes,mean operation stay were 3 5 days,postoperative bleeding was noted in 6 cases (16 7%) at the day of 2rd and 7th day of the operation,they were cured by conservative method.12 patients had no pain at the day of the operation.Follow up for 1~5 months the treatment of effective.Conclusion The new technique of PPH was a safe and effective treatment for third or fourth degree hemorrhoids and with the advantages of shorter time of inpatient stay and earlier return to normal activity,it may be an alternative for the conventional haemorrhoidectomy.
