摘 要:目的检测乙型肝炎病毒 (HBV)感染患者血清内HBVDNA量 ,探讨HBV复制水平与HBV免疫学标志物间关系。方法 492例乙肝病毒感染者血清用荧光聚合酶链反应作HBVDNA定量检测 ,35位正常人作对照。同时用固相放免法检测HBV感染的免疫学标志 ,统计不同免疫指标组合时HBVDNA检出率与含量。结果 35份正常人血清未检出HBVDNA ,46 2份HBV感染者血清中 ,311例HBVDNA阳性 ,检出率 6 7 32 %。其中HBsAg,抗HBc、HBeAg阳性血清 ,HBVDNA阳性检出率为96 0 6 % (195 / 2 0 3) ,平均HBVDNA含量 1 42× 10 8拷贝 /ml。HBsAg、抗 HBc、抗 HBe阳性血清 ,检出率5 0 85 % (90 / 177) ,平均含量 5 16× 10 6拷贝 /ml。HBsAg、抗 HBc阳性血清 ,检出率 40 98% (2 5 / 6 1) ,平均含量 3 18× 10 6拷贝 /ml。单项抗 HBc阳性血清 ,检出率 9 0 9% (1/ 11) ,其HBVDNA含量为 1 17×10 7拷贝 /ml。 6 1例HBsAg阳性、抗 HBc阳性血清作了HBcAg检测 ,发现HBVDNA阳性检出率与HBcAg呈正相关性。结论HBeAg与HBVDNA有很好的相关性 ,5 0 %抗HBe阳性 ,或e系统阴性患者病毒复制仍未停止 ,只是复制水平有所下降。HBcAg与病毒复制亦呈正相关性。荧光定量PCR检测血清HBVDNA含量有助于准确评价和判断HBV免疫标志物的临床意义。Objective To detect the level of HBV DNA in serum, to analyse the relationship between HBV immune markers and HBV DNA level and its clinical significant. Methods The HBV DNA was measured by a flurorescence quantitative polymerase chain reaction (FQ-PCR) in 492 patients with hepatitis B virus(HBV) infection and 35 normal adults. The sera samples were also tested for HBV immune markers by solid phase radioimmunoassay(SPRIA). Results Of the patients with HBV infection 67.32%(311/462) were HBV DNA positive. The normal adults were all negative. Among the 203 cases of HbsAg?anti-HBc?HbeAg positive, 195(96.06%) were positive for HBV DNA and the amount was 1.42×10 8 copise/ml on average. Among the 177 cases of HBsAg?anti-HBc?anti-HBe opsitive,90(50.85%) were positive with 5.16×10 6 copies/ml of HBV DNA. Among 61 cases of HBsAg, anti-HBc positive, 25(40.98%) positive and the amount of HBV DNA was 3.18%×10 6 copies/ml. The positive rate and amount of HBV DNA had obvious difference between the HBeAg positive group and anti-Hbe positive group. There was positive correlation between HBcAg and HBV DNA in observed objects. Conclusions FQ PCR can be used to monitor the true state of HBV infection and replication. The appearnce of anti-Hbe can not be taken as a sign of ceasing of HBV activity. The HBcAg may be used as clinical evaluation on HBV replication.
