摘　　要：目的 :观察鱼腥草注射液辅助治疗急性有机磷农药中毒 (AOPP)致呼吸肌麻痹 (RMP)并发呼吸肌相关肺炎 (VAP)的临床效果。方法 :94例AOPP致RMP并发VAP患者随机分为实验组和对照组 ,两组病人均给予头孢三嗪 2 .0g/ 12h静脉滴注 ,实验组在此基础上给予鱼腥草注射液 10 0mL静脉滴注 ,每日 1次 ,疗程 7d ,并在疗程中期和结束时比较二者的疗效。结果 :①疗程中期评价 :两组病人的肺部音、外周血白细胞变化相似 (P >0 .0 5 ) ,但实验组病人发热、呼吸道分泌物性状较对照组明显好转 (P <0 .0 5 ) ,3日无效淘汰率显著低于对照组 (P =0 .0 2 )。②疗程结束评价 :实验组痊愈率、总有效率和细菌消除率 (分别为 5 4.3 %、91.3%和 75 .7% )显著高于对照组 (分别为 30 .6 %、75 .0 5和 5 1.7% ) [P均 <0 .0 5 ]。③不良反应评价 :未见明显不良反应。结论 :鱼腥草注射液是一种安全有效的治疗AOPP致RMP并发VAP的辅助药物。Objective: To observe the clinical curative effects of Houtuynia cordata Injection as a adjuvant on ventilator-associated pneumonia (VAP) complicated with respiratory muscle paralysis (RMP) caused by acute organophosphorus pesticide poisoning (AOPP). Methods: 94 cases were randomly divided into two groups, all the patients were given 2.0 g of Ceftriaxone per 12 hours by intravenous drip for 7 days, on the basis of the above mentioned treatment, the patients in experiment group were given 100ml of Houtuynia cordata Injection per day by intravenous drip for 7 days. Then their curative effects were compared each other at the middle and end of period of treatment separately.Results: ① The evaluation at the middle of period of treatment: The variations of pulmonary rales and perpheral bloody white blood cells (WBC) of the two groups' patients were similar ( P >0.05), but the symptom of fever and character of respiratory tract's secretion of the patients in experiment group took turns for better obviously than the control group's ( P < 0.05 ), the rate of elimination for failed to respond to treatment for 3 days of the experiment group was obviously lower than the control group's ( P = 0.02 ). ② The evaluation at the end of period of treatment: The rates of recovery, total effectualness and elimination of bacterium of the experiment group (they were 54.3%, 91.3% and 75.7% respectively) were notably higher than the control group's (they were 30.6%, 75.0% and 51.7% respectively) [all P <0.05]. ③ The evaluation of the untoward reaction: There wasn't any obvious untoward reaction. Conclusion: Houtuynia cordata Injection is a secure and effectual adjuvant on RMP caused by AOPP complicated with VAP.
