慢性乙型肝炎肝脏病理与可溶性白细胞介素2受体关系探讨

摘　　要：目的　探讨慢性乙型肝炎病理与血清中可溶性白细胞介素 2受体 (sIL 2R)在慢性乙型肝炎发病机制中的作用和临床应用的价值。方法　采用双抗体夹心酶联免疫法检测了 30 7例病人血清sIL 2R(其中 2 75例慢性乙型肝炎和 16例脂肪肝均做了肝活检 ,16例慢性重型肝炎未做肝活检 ) ,用自动生化分析仪检测了血清总胆红素 (TB)、白蛋白(A)、球蛋白 (G)、丙氨酸转氨酶 (ALT)、碱性磷酸酶 (ALP)和γ 谷氨酰转肽酶 (γ GT)等指标在肝脏病理各级炎症及各期纤维化中的改变。结果　血清sIL 2R、TB、G、ALT、ALP和γ GT均值随着肝脏病理炎症程度加重而逐渐增高 ,A均值则随着肝脏病理炎症程度加重逐渐下降 ,各组间比较有显著性差异P <0 .0 5或P <0 .0 1。sIL 2R、G均值随着肝纤维化的进展而逐渐增高 ,A均值则随着肝纤维化的进展而逐渐下降 ,各组间比较有显著性差异P <0 .0 5或P <0 .0 1。结论慢性乙型肝炎病人血清sIL 2R、TB、A、G、ALT、ALP和γ GT水平的变化可作为判定肝组织炎症程度的参考指标 ,sIL 2R、A。Objective To investigate the relationship between the hepatic pathological changes and serum soluble interleukin 2 receptor(sIL 2R) level and evaluate the contribution of sIL 2 to the pathogenesis of chronic hepatitis B(CHB) and clinically validate this criteria. Methods Serum sIL 2R was detected in 307 patients (hepatic biopsy was done in the 275 CHB patients and the 16 fat liver patients, but not in the severe chronic hepatitis) with the method of monoclonal and polyclonal antibody ELISA. Serum level of TB、ALB、G、ALT、ALP、γ GT was detected with automatic biochemistry analysis machine in these patients with different degrees of hepatic inflammation and in different stage of hepatic fibrosis. Results With the increment of severity of hepatic pathological inflammation,overall level of serum sIL 2R 、G、ALT、ALP、γ GT increased gradually and that of ALB decreased otherwise. There was significant difference compared between each group ( P< 0.05 or P <0.01). Conclusion We conclude that serum marker sIL 2R 、TB、ALB、G、ALT、ALP、γ GT may be useful for determining the inflammation degree of the liver in CHB patient and sIL 2R、ALB、G may be referential indexes to indicate the degree of liver fibrosis.
