摘  要:目的 探讨咽鼓管表面不同活性物质 (ETS)在气压损伤性中耳炎 (BM)豚鼠模型咽鼓管 (ET)开放功能中的作用 ,以期为预防和治疗气压损伤性中耳炎提供理论依据。 方法 通过低压舱上升实验建立 BM动物模型 ;观察正常及分别应用人工 ETS、人工磷脂、咽鼓管灌洗液、生理盐水冲洗的 BM豚鼠 ET被动开放压 (POL)的变化。 结果  BM组 POL 为 11.91± 1.87k Pa,较正常组5 .96± 1.6 3k Pa明显增高 (P<0 .0 1)。BM组内 :人工 ETS使 POL 由 11.91± 1.87k Pa降低至 6 .6 7±1.72 k Pa,天然 ETS使 POL 由 11.98± 1.93k Pa降低至 6 .11± 1.6 9k Pa,均具有显著性差异 (P<0 .0 5 ) ,而人工 ETS与天然 ETS的活性作用无明显差异 ;对照的生理盐水使 POL 由 11.91± 1.82 k Pa变为 11.73± 1.6 2 k Pa,无显著性差异。而人工磷脂使 POL 由 11.86± 1.5 7k Pa降低至 8.6 1± 1.88k Pa,同生理盐水比较有显著意义 ,但同人工 ETS对 POL 的降低作用比 ,作用又明显减弱。 结论 人工 ETS同天然 ETS一样 。Objective To explore the effect of various surfactants on eustachian tube function in animals with barotitis media, so as to provide basis for prevention and treatment of the barotitis media in pilots. Methods Twenty guinea pig models of barotitis media were established with barocamera. The middle ear pressure needed to force eustachian tube (ET) to open (opening pressure level, POL) in normal guinea pigs and models of barotitis media were measured. Then the ETs of models were rinsed with natural eustachian tube surfactants (ETS), artificial ETS, phospholipid solution and saline respectively, and POLs were reexamined. Results The mean POL in normal guinea pigs was 5.96±1.63 kPa, and in models it increased to 11.91±1.87 kPa( P <0.01). After rinsing with natural ETS, POL of the models reduced from 11.98±1.93 kPa to 6.11±1.69 kPa, rinsing with artificial ETS, it decreased from 11.91±1.87 kPa to 6.67±1.72 kPa, rinsing with phospholipid solution from 11.86±1.57 kPa to 8.61±1.88 kPa, while rinsing with saline, from 11.91 ±1.82 kPa to 11.73 ±1.62 kPa. There were no significant difference between artificial ETS group and natural ETS group. Conclusion Artificial ETS is as effective as nature ETS in facilitating the opening of eustachian tube and aerating the middle ear.
