地氟醚麻醉对冠心病病人脑电图双频谱指数的影响

摘  要:目的 :研究 4种不同呼气末地氟醚浓度对冠心病病人血流动力学及数量化脑电图的影响。方法 :4 0例择期腹部手术病人 (ASAI -II级 ) ,用脑电监测仪监测不同呼气末地氟醚浓度下双频谱指数 (bispectralindex ,BIS)及相应平均动脉压、心率变化情况。结果 :地氟醚麻醉中BIS较基础值明显降低 (由基础值 91.4渐降至 4 4 .5 )(P <0 .0 5 ) ,且呈剂量依赖性 ;随着麻醉加深血压呈剂量相关性下降 ,在 1.0、1.5最低肺泡气有效浓度 (minimalalveolarconcentration ,MAC)分别较基础血压下降 14 .6 %、19.7% ;心率减慢 10~ 18次 /min(P <0 .0 5 ) ,但与剂量无相关性。结论 :地氟醚适宜于冠心病病人的麻醉 ,尤其在 1.0、1.5MAC可维持血流动力学稳定。BIS能反映麻醉深度 。Objective: To investigate effects of desflurane on the cardiovascular system and electroencephalogram in patients with coronary artery disease. Methods: Forty patients with coronary artery disease who were scheduled for elective abdominal operation were included. And at 0.5, 1.0, 1.5, 2.0 minimal alveolar concentration (MAC) points, BIS, MAP, HR were recorded respectively. Results: Desflurane anesthesia decreased MAP and BIS both in a dose dependent fashion(P<0.05).At 1.0, 1.5 MAC point, MAP decreased 14.6%, 19.7% respectively. As increasing of Des concentration, BIS decreased from 91.4 to 44.5 (P<0.05). HR decreased by 10~18 beats/min intraoperatively after inhalating Des, and no bradycardia occured in all patients. Conclusion: Desflurane is suitable for high risk patients and can maintain the hemodynamic stability at 1.0, 1.5 MAC during operation. BIS can be used as a parameter in judging the depth of anesthesia.
