摘  要:目的 探讨持续高通量静 -静脉血液滤过 (HV- CVVH)治疗多脏器功能障碍综合征 (MODS)的效果。方法 观察 6例 HV- CVVHF治疗 MODS患者治疗前后血流动力学、脉搏、血氧饱和度 (Sa O2 )和血浆尿素氮、肌酐、电解质及β2 MG变化。结果  HV- CVVH后 1h平均动脉压 (MAP)、 Sa O2 明显上升 ,心率明显降低 (P<0 .0 5 ) ;2小时后血钾、血磷、尿素氮、肌酐明显降低 (P<0 .0 5 ) ;4小时后 β2 微球蛋白开始下降 (P<0 .0 5 )结论  HV- CVVH可有效清除尿素氮、肌酐、 β2 MG,调整酸碱及电解质紊乱 ,改善血氧饱和度 ,使血流动力学稳定。Objective To explore the effect of highvolume continuous veno-venous hemofitration on multiple organ dysfunction syndrome.Methods The changes of blood flow kinetics,pulse,saturation of blood arterial oxygen(SaO 2)and serum biochemistry were monitored before and after the treatment of high-volume continuous veno-venous hemofitration in 6 patients with multiple organ dysfunction syndrom.Results The mean arterial pressure and SaO 2 were significantly raised,and the heart rate was decreased at 1st hour after hemofiltration(P<0.05).During hemofiltration,β 2MG were decreased at 4th.The serum potassium,phosphorus,urea nitrogen and creatinine were significantly decreased after hemofiltration(P<0.05).Conclusion HV-CVVH can effectively clear β 2MG,urea nitrogen and creatinine with improved SaO 2 and stable blood flow,and adjust the disorder of the balance between acid and base and electrolyte.
