肺、胸膜、纵隔实质性占位病变的超声显像及介入超声应用

摘　　要：总结分析肺、胸膜、纵隔实质占位病变的声像图特点 ,介入超声的方式、方法、注意事项及病理结果 .以超声检查后行超声引导下自动活组织切割检查的 5 8例肺、胸膜、纵隔实质占位病变为对象进行分析 .全部病例均获病理证实 ,其中肺部肿物 3 2例 ,胸膜肿物 16例 ,纵隔肿物 10例 .结果 :5 8例介入超声中 ,病理阳性结果者 5 3例 ,占 91 3 8% .其声像图特点 :①肺占位病变为周围型及实变肺中肿瘤 ,多随呼吸移动 ;②胸膜占位病变分弥漫型和局限型 ;③纵隔占位病变 ,后两者均不随呼吸移动 .声像图观察均可定位 ,但不能病理定性 .超声引导下穿刺活检能获得满意效果 .结果表明 :超声声像图特点 ,结合超声引导下自动活检可对肺外周型肿瘤及实变肺中的肿瘤、胸膜肿瘤、纵隔肿瘤作出准确的病理诊断 .The purpose of this paper is to summarize and analyze the characteristic of pulmonary, pleura and mediastinum substance with space occupying disease. The use of interventional ultrasound technique, the attention affairs and the pathological result were discussed here. 58 cases of pulmonary,pleura and mediastinum with space occupying disease which the slicer had been autoexamined by interventional technique were analyzed. All the cases had been testified in pathology. Out of the cases, 32 were pulmonary tumor, 16 pleura tumor, 10 mediastinum tumor. The results showed that out of the 58 cases examined by interventional ultrasound technique, 53 cases turned out to be positive in pathology, up to 91 38%.The characteristic of the ultrasonography showed: (1)Pulmonary space occupying lesion was brought by peripheral and solidation pulmonary tumor moving with breathe. (2)Pleura space occupying lesion included diffuse type and topical type. (3)The space occupying lesion did not move with breathe and the (2) situation didnt either. By means of ultrasonography, it could be located but could not be qualitatively analyzed in pathology. The satisfactory effect could be obtained by use of aspirationbiopsy leaded by ultrasonography and the confirmed diagnosis rate could be achieved. Conclusion:with the combination of the characteristic of ultrasonography and the auto-examination of the slicer leaded by the ultrasonography, the exogenetic pulmonary tumor, the pleura tumor and the mediastinum tumor could be diagnosed accurately in pathology.
