摘 要:目的 探讨β-纤维蛋白原 - 4 55G/ A(β- Fg- 4 55G/ A)基因多态性与环境因素对血浆 Fg表达的调节机制及与缺血性心脏病 (IHD)发病的关系。方法 应用 PCR及限制性内切酶分析的方法 ,分析 75例 IHD及 1 56例对照者 β- Fg- 4 55G/ A基因多态现象 ,比浊法测定血浆 Fg水平。结果 病例组血浆 Fg水平明显高于对照组 (P<0 .0 1 )。无论男、女患者与对照 ,A基因携带者血浆 Fg水平比同组 GG基因型者明显升高 (P<0 .0 5)。对照组中的 A基因携带者与病例组中的 GG基因型者 ,随增龄血浆 Fg水平有明显升高 (P<0 .0 5)。 IHD患者男性 ,按吸烟及基因型情况分组 ,相同基因型时 ,吸烟组血浆 Fg水平高于不吸烟与戒烟组 (P<0 .0 5)。病例组 A基因频率明显高于对照组 (P<0 .0 0 5)。结论 β- Fg- 4 55 G/ A等位基因与血浆 Fg水平相关 ,A基因携带者 IHD发病率更高。提示 β- Fg- 4 55G/ A可作为? Objective To investigate the influence of β Fibrinogen 455 gene polymorphism and environmental factors on fibrinogen expression and their relation with IHD. Methods The plasma fibrinogen levels were determined by Turbidimetry in all subjects (75 patients with IHD and 156 healthy controls).Theβ Fibrinogen 455 gene G→A mutation was identified by PCR and restriction enzyme digestion. Results The plasma fibrinogen levels were significantly higher in patients with IHD (3.73±1.0 g/L) than that in the controls (3.08±0.71 g/L) (P<0.01). Higher plasma fibrinogen levels were shown in the subjects with A 455 alleles. The r is 0.31 in patients and 0.18 in controls between fibrinogen levels and genotype. The plasma fibrinogen levels were higher in older person than in younger person in controls with A allele and patients with GG genetype (P<0.05), and in male IHD, the levels were significantly higher in smokers than in non smokers or ex smokers (P<0.05). The A allele frequency in patients was significantly higher than that in controls (P<0.005). Conclusion The β Fibrinogen A 455 allele is correlated with elevated fibrinogen levels in the patients or controls and those with A allele gene are prone to suffer IHD.
