外周动静脉同步换血换血量与胆红素水平关系的研究

摘　　要：目的　探讨高胆红素血症外周动静脉同步换血换血量与胆红素下降的关系。方法　对 2 5例高胆红素血症患儿采用外周动静脉同步换血 ,换血前 (0mL/kg)、换血 5 0mL/kg、10 0mL/kg和 15 0mL/kg时行胆红素系列检验。结果　高胆红素血症患儿换血后胆红素均有显著降低 ,总胆红素、间接和直接胆红素的换出率分别是46 1%、45 93 %和 44 6 4%。换血至第一个 5 0mL/kg时总胆红素换出率是 2 2 6 5 % ,第二个 5 0mL/kg时是14 94% ,第三个 5 0mL/kg时换出率是 8 5 1%。换血量 (X)与胆红素换出率 (Y)呈正相关 ,Y =3 2 6 5 +0 316 X,r =0 90 6 ,P <0 0 0 1。结论　总胆红素的下降与换血量成正比关系 ,但换出率逐渐减低。根据回归方程可预测推算所需要的换血量或换血后胆红素的水平。Objective To investigate the relationship between the bilirubin and exchange transfusion volume in hyperbilirubinaemia using peripheral vessels.Methods Exchange transfusion was performed through the peripheral vessels on 25 cases of hyperbilirubinaemia.Total serum bilirubin(TSB),indirect bilirubin(IBili) and direct bilirubin(DBili) were measured before starting(0mL/kg) and after exchanged 50mL/kg,100mL/kg and 150mL/kg.Results TSB,IBili and DBili were markedly decreased after exchange transfusion.The clearance rates of TSB,IBili and DBili were 46 1%?45 93% and 44 64%.The clearance rate of TSB was 22 65% after exchanging the first 50mL/kg,the second and the third 50mL/kg were 14 94% and 8 51%.The exchange transfusion volume(X) has perfect positive correlation with TSB(Y)(r=0 906,P<0 001)?The linear regression equation was:Y=3 265+0 316X.Conclusions The exchange transfusion volume has perfect positive correlation with TSB,but the clearance rates of TSB decreased gradually.Exchange transfusion volume and the concentration of TSB after finishing can be predication of using the linear regression equation.T5SS]AN Ji-fang AN Ji-fang (Party School of Shanxi Provincial Committee of C.P.C.,Taiyuan 030006,China) Abstract:In accordance with the spirits of the speech by Jiang Ze-min on July Ist, under the new historical conditions, we must maintain the Party's progressiveness and its purity, persist in integrating the fundamental principles of Marxism with the specific situation in China,persist in taking scientific theories as the guide, unswervingly take our own road, always rely firmly on the people, work sincerely for their interests and draw inexhaustible strength from them on the road of advance, consciously strengthen and improve Party building, continuously enhance the creativity, rallying power and combat capability of the Party and always maintain its vigor and vitality.
