白细胞滤器过滤晶体盐红细胞悬液进行血液质量与临床效果评估

摘　　要：目的　用国产滤器制备的去白细胞的晶体盐红细胞悬液 (Salineadenineglucosemannitolredbloodcellsuspension ,SAGMRBCs)进行质量和临床效果评估。方法　 2 0 0单位过滤后的SAGMRBCs,测定其过滤前后白细胞计数、红细胞计数、血小板计数、HCT、Hb、计算白细胞清除率、红细胞回收率、血小板损失率 ;1 5 0例受血者 ,输血前均不使用激素类及抗过敏类药物 ,观察输血不良反应。结果　经过滤去除白细胞的SAGMRBCs,白细胞清除率为 99 3 % ;红细胞回收率为 92 7% ,血小板损失率为 87 2 % ;在接受去白细胞的SAGMRBCs的 1 5 0例受血者中 ,输血≤ 2次的受血者中末发现输血不良反应 ,输血≥ 3次曾有过发热性输血反应史 (输注未去除白细胞的SAGM红细胞悬液 )的 3 0例受血者中仅有 1例出现发热性输血反应。结论　使用国产滤器能去除血液成分中99%以上的白细胞 ,能有效地预防非溶血性发热性输血反应。Objective To evaluate the blood quality and transfusion effects of the Saline adenine glucose mannitol red blood cell suspension (SAGM RBCs) after the management of domestic leukocyte filter. Methods RBC count, platelet count, haematocrit(HCT), hemoglobin(Hb), WBC clearance rate, RBC recovery rate, and platelet losing rate were detected in 200 units of SAGM RBCs before and after the filtration. The filtered SAGM RBCs was transfused to 150 patients without steroid or anti allergic drugs before transfusion and their reactions were observed for the side effects during and after the transfusion. Results The leukocyte clearance rate, red blood cell recovery rate, and platelet losing rate of the SAGM RBCs with domestic filter was 99 3%, 92 7%, and 87 2%, respectively. No side effects occurred in the 63 recipients with transfusion for 1 to 2 times. Among the 87 patents with ≥3 transfusion, there were 31 patients having had non hemolytic transfusion reactions (30 having fever and 1 allergic reaction) after the transfusion with non filtrated blood, but only 1 of them had fever and the other showed allergic reaction after leukocyte filtrated transfusion. Conclusion Domestic filter can filtrate about 99% leucocytes from SAGM RBCs and the filtration may effectively prevent non hemolytic febrile transfusion reactions.
