摘 要:目的 用 1%乙醇标记经尿道前列腺切除 (TURP)术灌洗液 ,研究呼出气乙醇浓度与血管内吸收量的关系。方法 2 5例TURP术患者 ,3%甘露醇膀胱灌洗液加乙醇 (终浓度 1% ) ,采用偶联酶法和Evan氏蓝染料稀释法测定呼出气乙醇浓度和血浆容量 ,以血浆容量变化值估计血管内吸收量 ,分析吸收量与呼出气乙醇浓度的相关性。结果 吸收量与呼出气乙醇浓度明显相关 (r =0 842 ,P <0 0 1) ,回归方程R2 =0 90 2 ,P <0 0 0 1;将冲洗时间加入多元分析 ,回归方程R2 =0 92 7,P <0 0 0 1。结论 乙醇标记监测法安全、简便、无创、无污染 ,与灌洗液吸收入血量相关性好 ,考虑时间因素的多元回归方程可用于临床定量监测TURP术灌洗液吸收。Objective To study the relation of the ethanol concentration in the expired breath (EB eth) and the intravascular absorbed volume of irrigation fluid(A vol) with 1% ethanol as irrigant tracer.Methods EB eth,continuous blood plasma volume variation,the volume of irrigating fluid entering and leaving the bladder,serum sodium concentration,and blood loss during operation were measured in 25 TURP patients with the irrigant containing 3% mannitol and 1% ethanol.Results There was a significant direct liner relationship between EB eth and the A vol gained from plasma volume measurement ( r= 0 842, P< 0 01).The linear regresion equation is A vol=99 5. EB eth 4 97(R 2=0 902, P< 0 001).The multiple stepwise regression equation is A vol=122. EB eth+0 72 Time 109 5(R 2=0 927, P< 0 001).Conclusion Ethanol monitoring is a safe,simple,non invasive,no pollution method of measuring irrigant absorption during TURP.The multiple stepwise regression equation taking absorption time into account is suitable for clinical quantitative monitoring of irrigating fluid absorption during TURP.
