纤维蛋白凝胶预防硬膜外粘连的实验研究

摘　　要：探讨纤维蛋白凝胶对脊柱外科术后硬膜外疤痕粘连的预防作用。方法 :2 4只新西白兔 ,行L4 、L5全椎板切除并去除硬膜外脂肪 ,在硬膜外分别覆盖纤维蛋白凝胶、游离脂肪瓣 ,并设立空白对照 ,术后 1个月和 3个月分批处死动物 ,取出手术段脊柱行大体观察和组织学研究。结果 :术后 3个月 ,纤维蛋白凝胶组的硬膜外疤痕组织少 ,疤痕与硬膜间仅有少量散在粘连。游离脂肪移植组 ,移植的脂肪块被纤维结缔组织包裹 ,硬膜囊前后均有粘连。空白对照组有较厚的疤痕增生 ,硬膜囊周围的粘连最为明显。结论 :纤维蛋白凝胶能减少实验动物椎板切除术后硬膜外疤痕形成和粘连。Objective:To investigate the usefulness of Fibrin glue as an interposition over the dura to prevent postsurgical epdrual scar adhesions.Methods:Wide lampinectomy was performed at L 4 and L 5 in 24 adult rabbits.The dura was covered with fibrin glue,free fat graft,or without intrpostition as a control,respectively.The operated column were obtained for gross and histological investigation.Results:In the fibrin glue group,epidural scar was thin and only a few adhesions were found around the drua after 3 months.In the fat graft group,the grafted fat were mostly shrunk and wraped by fibrous connective tissue.There were more adhesions around the dura.In the control group,thick fibrous connective tissue ws observed and adhesion around the exposed dura was the most apparent among the 3 groups.Conclusions:Fibrin glue is believed to be useful in reducing epidural scar adheesion after laminectomy.
