彩色M型多普勒超声心动图测定舒张早期血流传导速度评价左室舒张功能

摘　　要：目的 :彩色 M型多普勒超声心动图 (CMM)所测定的舒张早期血流传导速度 (FPV)评价左室舒张功能。方法 :使用 CMM所测定的 FPV与二尖瓣血流频谱和肺静脉血流频谱的相应数值进行比较 ,评价正常组 (2 8例 )、高血压组 (32例 )、冠心病组 (4 1例 )及心肌梗塞组 (2 9例 )的左室舒张功能的情况。结果 :(1)二尖瓣血流频谱表明高血压组与冠心病组左室舒张功能异常表现为松弛减退 (E/ A<1) ;(2 )心肌梗塞组中有 10例患者 E/ A>1,因此 E/ A值与正常组比较无明显差异 ,肺静脉舒张末逆流峰值 AR明显增大 ,FPV值明显减小 ,与其他三组比较均 P <0 .0 1,且每两组间比较也有差异 ,说明 AR与 FPV在区分假性正常方面有其独特的作用。单独比较正常 E/A >1组与心肌梗塞 E/ A>1组时 ,AR与 FPV的作用就更加明显。结论 :在评价左室舒张功能异常时 ,二尖瓣血流频谱能及早发现松弛减退 ,在二尖瓣血流频谱可能存在假性正常时 ,综合应用 AR和 FPV,可以对假性正常予以识别。考虑到经胸途径检查肺静脉往往比较困难且费时较多 ,而 FPV测定简便易行 ,是综合评价左室舒张功能的有用指标。Objective:To evaluate left ventricular diastolic function using transmitral,pulmonary venous flow velocity and early diastolic flow propagation velocity(FPV)measured by color M mode Doppler echocardiography.Methods:Transmitral pulmonary venous velocity(PV)and FPV were measured by transthoracic echocardiography in 22 hypertensive patients,41 coronary artery disease patients,21 post myocardial infarction patients and 24 control subjects.Results:(1)Transmitral flow velocity and E/A<1 in hypertensive patients and coronary artery disease patients while E/A>1 in control subjects and post myocardial infarction patients.(2)PV systolic to diastolic(S/D) ratio>1 in all group.The PV atrial reversal flow velocity(AR)was higher and the FPV was lower in post myocardial infarction patients compared with other groups(P<0 01).(3)AR and FPV in post myocardial infarction patients(E/A>1)had significant difference compared with those of the control group.Conclusions:Doppler echocardiography may appear as the principal clinical tool for the assessment of left ventricular diastolic function.Patients with hypertension and coronary artery disease typically exhibit a Doppler pattern of 'delayed relaxation'.AR and FPV may be useful in distiguishing a Doppler pattern of 'pseudonormalization'.
