摘  要:目的 :探讨高血压患者心肌不同程度肥厚背向散射参数变化的临床意义。方法 :根据左室重量指数(L VMI)将高血压患者分成三组 ,分别为组 :<12 5 g/ m2 (n =2 3) ,组 :12 5~ 174g/ m2 (n =2 6 ) ,组 :≥ 175 g/ m2 (n=2 2 ) ,分别测定各组心肌背向散射参数。结果 :室间隔与左室后壁部位三组 IBS%、CVIB存在显著性差异 (P <0 .0 5 ) ,而三组间 TGIB室间隔部位无显著性差异 (P >0 .0 5 ) ,左室后壁部位则存在显著性差异 (P <0 .0 5 )。室间隔厚度 (IVSd)与 IBS%、左室后壁厚度 (L VPWd)与其 IBS%之间均存在相关性 (r =0 .5 4,P <0 .0 5与 r =0 .5 1,P <0 .0 5 ) ;左室重量指数 (L VMI)与室间隔的 IBS%亦存在相关性 (r =0 .6 1,P <0 .0 1)。结论 :心肌背向散射参数可以用于评估高血压患者心肌肥厚的不同程度。Objective:To evaluate the clinical significance of myocardial backscatter parameters of hypertrophic myocardium in the patients with hypertension.Methods:Hypertensive patients were divided into three groups,group Ⅰ:<125g/m 2(n=23),group Ⅱ:125~174g/m 2(n=26),group Ⅲ:≥175g/m 2(n=22),according to left ventricular mass index(LVMI)and ultrasonic integrated backscatter(IBS)parameters were measured.Results:The values of percent integrated backscatter(IBS%)?cyclic variation of integrated backscatter(CVIB)of the three groups in the region of interventricular septum and left ventricular posterior wall were significantly different(P<0 05),and the values of transmural gradient of integrated backscatter(TGIB)of the three grous in posterior wall were different(P<0 05),but the value of that in interventricular septum had no signficant difference(P>0 05).The correlations were found between septal thickness and its IBS%(r=0 54,P<0 05),between posterior wall thickness and posterior IBS%(r=0 51,P<0 05),and between LVMI and septal IBS%(r=0 61,P<0 01).Conclusions:Ultrasonic integrated backscatter parameters may be used to evaluate to the degree of myocardial hypertrophy in the patients with hypertension.
