三尖瓣关闭不全的右心室造影诊断

摘　　要：本文探讨了临床体征、胸部平片和右心室造影对风湿性心脏病二尖瓣病变患者功能性三尖瓣关闭不全的诊断价值。共作右心室造影39例,其中27例作了心脏直视手术。结果表明体征和胸片可提示诊断,但不敏感,也不可靠,右心室造影特别是改进法在三尖瓣反流的分级方面与手术结果相关性好(r=0.92,P<0.01),符合率高(85%)。右心室和三尖瓣环的扩大为反流的主要机制。The values of clinical signs,chest plain film and right ventriculography in the diagnosis of functional tricuspid regurgitation(TR)in patients with rheumatic heart disease were evaluated.Right ventriculography was performed in 39 patients and 27 of them underwent open heart surgery.It was found that clinical signs and chest film were suggestive but unreliable in diagnosing TR.Our modified right ventriculography correlated well with surgi- cal exploration(r=0.92,P<0.01)in determining the degree of TR,having a high rate of accordance(85%).Dilatation of the right ventricle and the orifice of tricuspid valve may be the principal mechanism of TR.
