摘　　要：目的　探讨脉冲振荡法 (IOS)测定指标在COPD的特征性改变及临床意义。方法　测定、分析 16 5例健康人 ,95例COPD患者的IOS指标 (Rf、Zrs、R5、R2 0、R35、X5、Rc、Rp) ,同时测定传统通气功能 (VC、FEV1.0 、FEV1.0 %、MMEF、MVV)。IOS结果作t检验。结果　COPD组与健康组比较 ,IOS各项指标均明显增高 (P <0 .0 1)。其中以共振频率(Rf)、周边气道阻力 (Rp)、呼吸阻抗 (Zrs)、气道总阻力 (R5 )、弹性阻力 (X5 )明显高于中心气道阻力 (R2 0Rc)及上气道阻力 (R35 )。COPD组IOS测定结果输出图形具有特征性改变。结论　IOS方法能较好地全面反映慢性阻塞性肺部疾病呼吸生理的改变 ,有较大的临床肺功能诊断价值 ,值得推广应用 ,但不能替代传统肺功能而应互补结合则为更优方法。Objective To discuss the clinical significance and change of characteristic of impulse oscillometry(IOS) in chronic obstructive pulmonary disease(COPD).Method The IOS indices of 165 normal cases and 95 cases of COPD were measured and traditional lung function(VC,FEV 1.0 ,MMEF,FEV 1.0 %,MMV) were measured at the same time. t test was pursued with the IOS.Results Compared with normal group,the IOS indices of COPD rose obviously( P <0.01).In resonant frequency(Fres),peripheral airway resistance(Rp),impedance(Zrs),total airway resistance(R5) and peripheral capacitance(X5) were significantly higher than central airway resistance(R20),central resistance(Rc) and the upper airway resistance(R35).There was characteristic change of IOS in COPD.Conclusions IOS can reflect overall variation of respiratory physiology in patients with COPD.It is more useful to lung function in clinical diagnosis.It can be spread and applicable.But it can't replace traditional lung function.
