经内镜鼻胰管引流术在非胆源性急性重症胰腺炎早期治疗中的应用

摘　　要：目的 :探讨经内镜鼻胰管引流术在酒精性等非胆源性急性重症胰腺炎早期治疗中的应用价值。方法 :17例患者诊断后随机分为治疗组和对照组 ,治疗组立即在十二指肠镜及X线下放置鼻胰引流管 ,鼻胰管引流同时还使用相关药物。对照组按常规内科方法处理。详细观察症状、体征、实验室及CT指标变化、并发症发生情况和住院天数。结果 :治疗组患者症状、体征、白细胞计数、胰外渗液等改善时间明显短于对照组 ,住院天数也显著减少 (P <0 .0 1) ,且发生并发症少 ,程度轻。结论 :鼻胰管引流术治疗非胆源性急性重症胰腺炎具有肯定的疗效优势 。Objective:To evaluate the early treatment application of endoscopic naso-pancreatic drainage(ENPD)in serious acute pancreatitis which is not due to cholangioens. Methods:Seventeen cases were divided into two groups randomly:the therapeutic group(treated with ENPD,as well as drugs) and the control group (treated with drugs only),comparing the difference of symptoms, signs,scores of leucocytosis and CT,the occurence of complications,the day cost in hospital.Results:The progress of symptoms,signs,scores of leucocytosis and effusion around pancreas were better in therapeutic group than that in control group.There were significant differences of the degree of complications,the days cost in hospital between two groups. Conclusions:ENPD has undoubted therapeutic value in SAP not due to cholangioens, and should be used in those couldn't skilled apply EST.
