摘 要:目的 螺旋CT三维重建技术重建前庭小管外口 ,探讨螺旋CT三维重建技术的临床应用价值。方法正常耳组 30例 (6 0耳 )。梅尼埃病组 30例 (37耳 )均采用螺旋CT扫描 ,把获得的数据处理后传至Sun工作站 ,采用表面成像法、切割技术及沿X轴、Z轴的图像旋转技术进行三维重建。结果 所有受检者前庭小管外口均获得清晰显示。分别测得其宽径 ,正常耳组 (6 0耳 )前庭小管外口宽度为 (6 6 2± 1 6 9)mm ,梅尼埃病人组 30例 (37患耳 )前庭小管外口宽度为 (3 91± 1 17)mm ,其差异具有显著性意义 (P <0 0 1)。结论 螺旋CT三维重建技术可以重建前庭小管外口 ,并可直接测得其宽度。结果表明 :正常前庭小管外口宽度大于梅尼埃病人患耳 ,前庭小管外口狭窄 ,可能是梅尼埃病的病理基础 。Objective To study the clinical significance in the external aperture of vestibular aqueduct(VA) with using threer-dimensional (3D) reconstruction technique.Methods The experiment consisted of two groups,normal ear group(30 cases,60 ears)and Miniere's Disease(MD) group(30cases,37ears).Xvision-GX spiral CT scanning was performed in all the subjects,the data obtained were treated in Sun-Xtension work station for 3D reconstruction,with shaded surface display technique,clip technique and image-rotation round S and Z axes.Results All external apertures of vestibular aqueducts were revealed clearly,and its width was then measured directly on the image itself.The width of external aperture in 60 normal ears was(6.62±1.69)mm,and ( 3.91 ±1.17)mm in 37 ears with MD.The difference was highly significant (test was used, P <0.01 ).Conclusion Spiral CT-3D reconstruction technique may reconstruct the external aperture of VA,and measure its width directly.The results of measurement show that the width of external aperture in 37 ears with MD was narrower than that in 60 normal ears.It may be one of the fundamental pathological anatomy in MD.So measuring the external aperture of VA in 3D image was very important for dia gnosis and differential diagnosis of MD.
