柔性复位固定器治疗小儿股骨干骨折的临床研究

摘　　要：目的　研究治疗小儿股骨干骨折 ,尤其是上 1/ 3骨折 ,复位后稳定性差 ,易造成前外成角而畸形愈合的有效方法。方法　治疗组 168例采用自制柔性复位固定器 ,通过滑动压板的持续压力 ,对抗髂腰肌、臀肌、内收肌等造成不稳定及成角趋势。对照组 5 0例采用传统的骨牵引结合手法复位小夹板外固定术。结果　参照Ogden标准 ,治疗组 168例中优 13 6例 ,可 2 9例 ,差 3例。对照组 5 0例中优 2 3例 ,可 16例 ,差 11例 ,差异有显著性。结论　柔性复位固定器治疗小儿股骨干骨折 ,尤其是上 1/ 3骨折 ,防止前外成角畸形效果理想。Objective To find out an effective method for the treatment of femoral shaft fracture in the infants.These fractures unstable after closed reduction and are easilly to have anterior lateral angulation deformity Methods 168 cases were treated with flexible reduction fixation apparatus,and through the constant pressure of slide clamp to countereact the unstablility and tendency of angulation caused by iliopsoas,gluteal and adductor muscles were checked.50 cases in control group were treated with traditional traction and splint immobilization.Results In accordance with Ogden standard,the results were 136 in good,poor in 29 and 3 fair in treatment group.Of 50 cases in control group,23 good,16 poor and 11 fair.There were significant difference between them.Conclusion Flexible reduction fixator should be recommended for the treatment of infantile femoral shaft fracture espically on 1/3 part to prevent anterior lateral angulation.
