慢性重型肝炎患者肠道菌群、血浆内毒素相关性研究与微生态制剂的调节

摘　　要：目的研究慢性重型肝炎患者肠道菌群、血浆内毒素变化规律以及两者之间的相互关系 ,探讨微生态制剂的调节机制。方法 :取 5 0例慢性重型肝炎患者 0 .5g新鲜粪便按 10倍稀释法稀释致 10 -10 ,用滴注法接种于选择性培养基上培养 ,选择肠道中具有代表性需氧菌和厌氧菌共 8种。采用基质偶氮显色法实验定量测定外周血内毒素。给其中 30例口服整肠生胶囊 ,4粒 /次 ,3次 /d ,共 30d。结果 :5 0例拟杆菌、双歧杆菌、乳杆菌明显低于健康组 (P <0 .0 1) ;血浆内毒素明显高于健康组 (P <0 .0 0 1) ,与杆菌减少呈正相关关系 ;口服整肠生后 ,内毒素明显下降 (P <0 .0 1) ,肠道中杆菌增加 (P <0 .0 1)。结论 :慢性重型肝炎患者存在肠道菌群失调和高内毒素血症 ,肠道微生态制剂能够调整肠道失调菌群 ,降低血浆内毒素 。Objective:In order to investigate the relationship of bacterial flora and endotoxemia as well as regulating effects and mechanism of microecological preparation in patients with chronic sever hepatitis. Methods:Stool samples of 0.5 gram from 50 patients with chronic sever hepatitis were diluted in series by 10 times to 10 -10 ,and then cultured in selective culture medium by which 8 species both anaerobic and aerob were selected.Endotoxin in plasma was examined with luminous test.The above examinations for 30 patients were carried out before and after 4 capsule zhengchangsheng,a microecological preparation,which were taken at 3 times every day for 30 days.Results:Compared with normal controls the bacillus such as bacillus similus,bifidus and lactis were reduced while plasma endotoxin increased significantly in the 50 patients.Closed relationship existed between the decrease of the bacillus and the increase of plasma endotoxin.The abnormal changes were improved after administration of microecological preparation in the 30 patients.Conclusions:The disturbance of bacterial flora in the digest tract and high endotoxemia exist in patients with chronic sever hepatitis.Capsule Zhengchangsheng showes therapeutic effects on the treatment of this disease.
