梗死心肌的超声背向散射积分参数研究

摘　　要：目的 :研究超声背向散射积分 ( IBS)定量评价梗死心肌组织特征的临床价值。方法 :对 3 5例各期不同部位心肌梗死患者 (梗死组 )及 3 5例正常人 (对照组 )分别测定相应部位心肌及心包的背向散射积分参数 ,且受试者均经冠状动脉造影证实。结果 :梗死心肌的 IBS标化值、IBS周期变异延迟校正值 ( N- delay)较正常心肌显著增高 ( P<0 .0 0 1) ,而 IBS周期变异幅度及周期变异率显著减低 ( P<0 .0 0 1) ,IBS跨壁差趋于消失。结论 :心肌的超声背向散射积分参数能定量反映梗死心肌的组织特征 ,为心肌梗死的诊断提供重要依据。Objective:To evaluate the clinical signific anse of qu antitative analysing histological characteristics of infarcted myocardium by ult rasonic integrated backscatter.Methods:Integrated backscatter (IBS) on line analysis was carried in 35 patients with recent or old myocardial infarctio n and 35 normal persons. Coronary arteriographies were performed in all subjects . Results:There were significant differences in calibrated myocardia l in tegrated backscatter, magnitude of cyclic variation in integrated backscatter, r atio of CVIB and calibration of delayed time of CVIB (N delay) between infarcte d and normal myocardium, and the transmural gradient of IBS disappeared in infa rcted myocardium. Conclusions:Analysing of integrated backscatter p ara meters can quantitatively assess the tissue characterization of infarcted myocar dium, it is useful on the diagnosis of infarcted myocardium. [
